| ObjectiveTo develop and establish initial reliability and validity of the Colostomy Patients Colostomy Knowledge,Attitude and Practice Scale(CPCKAPS),to provide appropriate evaluation tools for medical staff.To investigate the condition and the influence factors of Colostomy knowledge,attitude and practice of colostomy patients,in order to improve the effect of health education and provide a theoretical basis for colostomy patients.Methods1.Qualitative research:through literature retrieval,books and research group members to discuss the formation of KAP dimensions item pool and interview outline of colostomy,a total of 5 wound stoma therapists and a total of 10 patients with colostomy were selected by using the method of purposive sampling in the 3 hospitals of grade three in Nanchang,and they were interviewed on the basis of the interview outline.Adjust the item pool according to the interview results,the formation of the first round of inquiry questionnaire.2.Experts consultation and reliability and validity test:engaged in 23 experts who were work on personnel management and nursing work of wound ostomy care as the scale construction expert.Through 2 rounds of expert enquiry were initially formed "intestinal colostomy knowledge,attitude and behavior scale".The preliminary questionnaire was used to facilitate the pre survey of 132 patients with enterostomy in the 3 hospitals of grade three in Nanchang,The pre survey results were analyzed by item analysis,correlation analysis and factor analysis and then the reliability and validity of the scale were tested to form the final knowledge,attitude and behavior scale of colostomy patients.3.Survey of colostomy patients:208 cases of patients with colostomy in Wound stoma clinic and inpatinent department of Nanchang city were selected byconvenience sampling method,which were selected from 3 hospitals of grade three,the general situation questionnaire,colostomy stoma knowledge,attitude and behavior scale(CPCKAPS)were used to investigate the status of colostomy patients and analysis its influence factors.4.Statistical methods:Using Excel2013 to establish the database,using SPSS22.0 statistical software for data analysis.All tests were two-sided test,with P<0.05 as the difference was statistically significant.Measurement data using the mean±standard deviation,the number of data used to describe the frequency and composition ratio,The two independent samples t test,one-way ANOVA,multiple linear regression analysis were used to evaluate the influencing factors of colostomy patients' level of KAP.Results1.Qualitative research results:Qualitative research results of intestinal colostomy knowledge,attitude and behavior of the 3 dimensions of a total of 50 entries;the colostomy knowledge dimensions of 17 items,stoma attitude dimensions of 20 items,13 items of stoma behavior dimensions.2.Delphi expert consultation and reliability and validity test results:The active coefficient of the two rounds of expert consultation were 100%,the number of experts to advise the ratio of 65.22% and 13.04%,respectively,the 2 round of expert consultation authority coefficient were 0.876,the 2 round of expert consultation questionnaire of general kendall coefficient of concordance were 0.203 and 0.127,the difference was statistically significant by chi square test(P<0.05),which means that it has a good coordination,the initial formation of colostomy stoma knowledge attitude behavior scale,including 3 dimensions and 43 items.By analyzing the results of the preliminary investigation,3 items were not deleted,the pre research results showed that the total Cronbach's a coefficient of the questionnaire was 0.874,and the split half reliability coefficient was 0.767,The coefficients of Cronbach's a were 0.731,0.833 and 0.810,respectively;The split half reliability coefficients were 0.632,0.729 and 0.767,respectively;The level of each item of the questionnaire was 0.869~1.000,and the level of CVI was 0.982.Finally,to determine the stoma of the colostomy patients,the final scale was 15 items of knowledge dimension,15 items of attitude dimension,10 items of behavior dimension.3.Analysis of KAP level and influencing factors in patients with enterostomy:The total score of KAP was(76.01±6.20)in patients with colostomy,and the highest score was the dimension of attitude of colostomy(42.11±4.58),and the lowest dimension was knowledge of colostomy(8.72±2.06).Multiple linear stepwise regression analysis showed that the main influencing factors of colostomy patients were patients,age,education level,residence place,economic income,the time after enterostomy,the number of patients attending the meeting,the frequency of the home of the sun and the return visit of the stoma.ConclusionsThe knowledge,attitude and behavior scale of colostomy patients had good reliability and validity,and could be used to evaluate the level of KAP of colostomy patients.Colostomy related knowledge is not optimistic,attitude is positive,behavior is not ideal,The main influencing factors of the patients with colostomy sstoma of KAP were age,education level,residence place,economic income,the time after enterostomy,the number of patients attending the meeting,the frequency of the home of the sun and the return visit of the stoma.Therefore,nursing staff should strengthen the education of patients with colostomy stoma knowledge,and guide the patients to masmaster the knowledge used in daily care.At the same time,we should focus on different factors to guide,improve the effect of health education,so as to improve the quality of life of patients... |
