Construction Of Competence-based Disaster Nursing Training Curriculum

ObjectiveThis research develops a disaster nursing competency index system that meets China's characteristics to clarify the knowledge and ability elements that nurses need in disaster relief.Then constructs a competence-based disaster nursing training curriculum.The contents,teaching strategy,class hours and assessment methods of the curriculum are discussed to provide theoretical reference and practical guidance for systematic and standardized disaster nursing training.MethodsThis research uses a variety of scientific research methods such as literature analysis,Delphi method,and questionnaire survey method to study the issues of disaster nursing competency and training courses.Firstly,through extensive reading and analysis of domestic and foreign literature related to disaster nursing competency and curriculum,the basic concepts involved are defined,the theoretical basis is established,and the disaster nursing competency indicator system is initially formulated.Secondly,through selecting representative and authoritative experts in the field of disaster nursing or disaster medicine,conducting expert consultation on the proposed disaster nursing competency indicator system,the indicator system is improved continuously according to expert opinions until the expert opinions tend to be consistent to establish the indicator system.The hierarchical analysis is applied to test the scientific nature of the established indicator system structure,and the weights of indicators at all levels are set.According to the indicator system,the questionnaires are conducted to pre-survey the nurses' disaster-care ability.Through the analysis of the pre-survey results,the indicators are screened,and the scientific and reliability of the indicator system is tested by the mathematical and statistical methods such asreliability and validity analysis.In addition,the objectives of the disaster nursing training course are established through literature review and theoretical analysis.Focusing on the course objectives,the research team built a preliminary plan for disaster nursing training curriculum based on the preliminary proposed disaster nursing competency indicator system,including curriculum elements,teaching strategies and assessment methods.According to the preliminary curriculum plan,an expert consultation questionnaire is prepared to conduct expert consultation on the course content.Meanwhile,the teaching methods are set as multiple-choice questions,the number of class hours and assessment methods are set as open-ended questions to consult experts suggestions.A curriculum plan is established after two rounds of expert consultation.Based on the established curriculum plan,the pre-survey questionnaire for nurses' disaster nursing training needs is compiled.The results of the questionnaire were analyzed by scientific statistics such as project analysis,reliability and validity test to test the scientific nature and reliability of the questionnaire.Results1.The constructed disaster care competency indicator system consists of three first-level indicators,eleven second-level indicators and fifty third-level indicators,and the structure of the indicator system is scientific and reasonable.The weights of the first-level indicators are 0.3119 for knowledge system,0.4905 for professional ability,and 0.1976 for comprehensive quality.The weights of the secondary indicators are 0.1040 for disaster nursing basics,0.2079 for disaster nursing expertise,0.1622 for assessment and observation,0.0876 for non-emergency emergency response,0.1181 for on-site first aid,0.0387 for disaster nursing management,and 0.0303 for psychological crisis intervention,0.0535 for communication and collaboration ability,0.0659 for physical quality,0.0659 for psychological quality,0.0659 for ideological and moral quality.2.The disaster nursing training course is composed of two modules and sevenparts,totally thirty-four specific contents.The two modules are theoretical training module and skill training module.In detail,the theoretical training module consists of five parts: disaster nursing basic knowledge,disaster nursing professional knowledge,disaster nursing management knowledge,disaster psychological crisis intervention,and communication and cooperation in disaster relief.The skill training module consists of two parts: special technology training and first aid skill training.The various sections are subdivided into specific training content ranging from two to nine items.The course consists of twenty-three hours of instruction,including theoretical lectures,case studies,scenario simulation exercises,discussions,role play,video watching,and operational skills training.The assessment methods include theoretical assessment and operational skills assessment.3.The positive coefficients of experts in the two rounds are 75% and 100%respectively,and the authoritative coefficients of experts are 0.818.In the first round of inquiry for disaster care competency indicator system,the importance,standard deviation,full rate,selection rate range are 4.07 to 4.93,0.26 to 0.88,40.00 to 93.33%,66.67 to 100.00% respectively,the coefficient of variation is 5.27 to 21.62%,and the Kendall coordination coefficient is 0.172(P<0.001).A total of eleven experts put forward forty opinions,thirty-two of which are adopted.In the second round of inquiry,the importance,standard deviation,full rate,selection rate range are 4.40 to5.00,0.00 to 0.83,26.67 to 100.00 and 80.00 to 100.00% respectively,the coefficient of variation is 0.00 to 18.94%,and the Kendall coordination coefficient is0.240(P<0.001).One expert puts forward two opinions,all of which were adopted.After the pre-survey,the results of project analysis are good,and no indicator is deleted.The reliability and validity test results show that the total content validity of the scale is 0.95,and the content validity of each item is 0.80 to 1.00;the correlation coefficient between the first level indicator and the indicator is 0.810 to 0.982,and thesecondary index and the first level indicator The correlation coefficient is 0.394 to0.976;the total Cronbach's ? coefficient of the scale is 0.984,the Cronbach's ?coefficient of each field is 0.929 to 0.981;the total half-fold reliability of the scale is0.920,and the half-fold reliability of each field is 0.874 to 0.954.4.In the first round of inquiry for disaster nursing training course setting,the importance,standard deviation,full rate,selection rate range are 4.13 to 5.00,0.00 to0.91,46.67% to 100.00%,and 86.67% to 100.00% respectively,the coefficient of variation is 0.00 to 20.68%,and the Kendall coordination coefficient is0.195(P<0.001).A total of ten experts put forward twenty-seven opinions,twenty-three of which are adopted.In the second round of inquiry,the importance,standard deviation,full rate,selection rate range are 4.13 to 4.93,0.26 to 0.88,33.33% to 93.33%,and 73.33% to 100.00% respectively,the coefficient of variation is 5.27 to 20.61%and the Kendall coordination coefficient is 0.194(P<0.001).One expert puts forward two opinions,all of which are adopted.The results of the pre-survey project analysis of nurses' disaster nursing training needs are good,and no indicator is deleted.The reliability and validity test results show that the content validity index of the questionnaire level is 0.93,the content validity index of the item level is 0.80 to 1.00;the correlation coefficient between the first level indicator and the indicator total is 0.973,0.975,respectively.The correlation coefficient of the index is 0.779 to 0.971;the total Cronbach's ? coefficient of the questionnaire is 0.991,the Cronbach's ? of each field is 0.983 to 0.988;the total half-fold reliability of the questionnaire is 0.958,and the half-fold reliability of each field is 0.933 to 0.965.ConclusionThis research comprehensively applies a variety of research methods,and builds a scientific and reliable disaster nursing competency indicator system,which lays a theoretical foundation for the evaluation,selection and cultivation of disaster nursingtalents,and provides a reference basis.Based on the competency,a practical and operative disaster nursing training course is constructed.The training course content,teaching strategy,class hours and assessment methods are discussed.It provided theoretical reference and practical guidance for systematic and standardized disaster nursing training.
