Objective:To investigate the present situation of the quality of continuous nursing service in hospitals and the factors affecting the quality of continuous nursing service,and to understand the demand and influencing factors of continuous nursing service for diabetic patients after discharge from hospital.To analyze the correlation between the continuous nursing needs of diabetic patients and the quality of continuous care,and to provide guidance for the development of continuous nursing services guided by the needs of patients in the future?Methods:1.412 diabetic patients hospitalized in a tertiary A hospital in Xinjiang from January to July 2018 were selected as subjects by cross-sectional survey.A questionnaire survey was conducted among the patients on the day of discharge by using the Chinese version of the continuous Nursing scale(care transitions measure,CTM)and the continuous Nursing needs scale for Diabetic patients.Two hundred and sixty-six patients were followed up by telephone 2~4 weeks after discharge from hospital with CTM and continuous care needs scale for diabetic patients.2.The data were inputted by EpiData 3.1.The data were analyzed by SPSS17.0 and IBM AMOS 17.0statistical software.Cronbach's?coefficient and factor analysis were used to evaluate the reliability and validity of the scale.Before evaluating the validity,KMO test and Barley sphericity test were performed.Descriptive statistics were used to analyze the general data and disease data.the counting data were described by frequency and percentage,and the measurement data were described by mean±standard deviation.T test or ANOVA was used to analyze the data,multivariate linear regression was used to analyze the meaningful variables in univariate analysis,and rank sum test was used to analyze the data that did not obey the normal distribution.The test level is?=0.05.Results:1.Reliability and validity analysis:three dimensions were designed according to the purpose of the study.the lowest Cronbach's?coefficient of each dimension was 0.787,while the Cronbach's?coefficient of the total scale was 0.881.The reliability of the scale is good and the internal consistency among the items is good.The KMO value was 0.881,the Bartlet?s spherical test chi-square value was 5952.962(P<0.01),and the degree of freedom was 0.210.The results of model goodness of fit test showed that?~2/DF=2.306,less than 3.0,RMSEA?0.069,less than 0.08,CFI(0.955),NFI(0.935),TLI(0.929),IFI(0.956)were all within acceptable standards.On the whole,the scale model has a good goodness of fit in diabetic patients,that is,the theoretical model structure fits the empirical data,suggesting that the three dimensions can better reflect the continuing care needs of diabetic patients.2.The total score of continuous nursing quality of diabetic patients was(65.10±19.50),the score of general self-care preparation dimension was(79.27±17.68),and the score of doctor-patient communication dimension was(69.25±19.265).The score of health monitoring dimension was(65.55±20.44),and that of written plan was(46.33±20.63).There were significant differences in the effects of age,type of medical insurance,personal monthly income and previous diabetic health education on the quality of continuous care for diabetic patients(P<0.05).3.The total score of continuous care needs of diabetic patients was(74.021±11.20),the score of diabetic disease nursing dimension was(78.84±15.51),and the score of complication nursing dimension was(72.62±18.00).The score of health promotion dimension was(72.42±10.86).The age,economic source and medical insurance of the patients were the main factors affecting the continuous care needs of diabetic patients(P<0.05).4.The quality of continuous nursing care of diabetic patients was negatively correlated with the demand of continuous nursing care of diabetic patients.Conclusion:The quality of continuous care for diabetic patients needs to be improved,especially the provision of appropriate written materials.The age of the patients,the type of medical insurance,personal monthly income and whether they have received diabetes health education in the past are the main factors that affect the quality of continuous nursing care for diabetic patients.The demand for continuous care of diabetic patients is in the middle level,and hospitals should carry out continuous nursing services guided by the needs of patients.
