Meta-Analysis Of The Efficacy Of Post-auricular Injection Of Methylprednisolone And Systemic Glucocorticoids In The Treatment Of Sudden Deafness

Objective To analyze the efficacy of post-auricular injection of methylprednisolone and systemic glucocorticoids in the treatment of sudden deafness by Meta-analysis.Methods The databases retrieved included Chinese and English databases,such as CNKI,WanFang,VIP,CBM,PubMed,EmBase,Cochrane,etc.,involving the literature on the treatment of sudden deafness with post-auricular injection of methylprednisolone from database setup till June 2018.An objective quality assessment was performed on the related literature,and a Meta-analysis was performed using Rev Man 5.3 software.Results A total of 11 literatures were included in the analysis.Meta-analysis showed that the test group was better than the control group in the treatment of sudden deafness [OR=3.18,95% CI 2.13~4.76,P<0.00001].Subgroup analysis showed that the control group were divided into group A,group B and group C according to systemic glucocorticoids.In the three groups,the treatment effect of the test group was superior to that of the control group [OR=2.73,95% CI 1.56~4.78,P=0.0004?OR=3.59,95% CI 1.45~8.92,P=0.006?OR=3.89,95% CI 1.82~8.31,P=0.0004].The treatment effect of the test group was better than that of thecontrol group in the treatment of sudden deafness of total deafness type[OR=4.07,95% CI 1.55~10.68,P=0.004].The improvement of hearing in frequency bands of 125-250 Hz and 125-500 Hz was significantly higher in the test group than in the control group [MD=15.25,95% CI 13.18~17.32,P<0.00001?MD=14.90,95% CI 13.06~16.74,P<0.00001].The hearing improvement in the three frequency bands of 500-2000 Hz,4000-8000 Hz and 250-4000 Hz showed no obvious advantage in the test group compared with the control group [MD=6.49,95% CI-0.37~13.35,P=0.06?MD=8.72,95% CI-1.08~18.52,P=0.08?MD=7.59,95% CI-0.25~15.43,P=0.06].Conclusion The effect of post-auricular injection of methylprednisolone in the treatment of sudden deafness is superior to systemic glucocorticoids.However,due to the lack of foreign literature,the quality of the included literature is not high.The conclusions drawn from this meta-analysis need further investigation.
