Research purposeThis study retrospectively collected the clinical data of patients with primary dysmenorrhea who were treated with traditional medical comprehensive therapy,summarized them,and strived to obtain the research results which could be easily translated into clinical practice,so as to provide reference for the comprehensive treatment of primary dysmenorrhea withtraditional medical comprehensive therapy.Expanding the research methods of primary dysmenorrhea to provide valuable clues for further research.Research methodsBy landing outpatient cases and case system,527 PD patients who met the criteria of inclusion and exclusion in this study were collected and Epidate database of comprehensive treatment of primary dysmenorrhea was established.The age,menarche age,course of disease,syndrome type,first visit prescription and external treatment,NRS score before treatment,NRS score after 3 menstrual cycles and adverse reactions were extracted.SPSS 22.0 was used to analyze the data by frequency analysis,cluster analysis,factor analysis and other statistical methods for the relevant data in the database.The results of the research(1)Distribution of syndrome types in 527 PD patients in this study: Qi stagnation and blood stasis syndrome(34.3%),cold coagulation and blood stasis syndrome(29.4%),liver and kidney deficiency syndrome(14.0%),Qi and blood deficiency syndrome(12.3%)and damp-heat stasis syndrome(9.9%).The NRS scores of eachgroup after treatment were lower than those before treatment.No adverse reactions were recorded.(2)Sixty-two kinds of prescriptions were investigated,and the most frequently used prescriptions were self-made prescriptions(34.9% of the total).The number of times of using traditional Chinese medicine was 6177,involving 209 kinds of drugs.The clustering analysis of 66 traditional Chinese medicines with frequency over 20 times showed that the strongest clustering drugs were Rhizoma Trigoni,Rhizoma Curcumae,Rhubarb and Panax Notoginseng powder,and 18 common factors were extracted by factor analysis.(3)External treatment involved 15 categories.Comparisons in syndrome differentiation and typing groups: in Qi stagnation and blood stasis group,two external treatments were used most frequently,ironing frequency of traditional Chinese medicine was the highest,followed by acupoint application;in cold coagulation and blood stasis group,two external treatments were used most frequently,moxibustion was the highest,and acupoint application was the second;in liver and kidney deficiency group,one external treatment was used the most,moxibustion and ironing frequency of traditional Chinese medicine was the highest;in Qi and blood deficiency group,one external treatment was used the most,and moxibustion frequency was the highest.In the dampness-heat-stasis group,one external treatment was used most frequently,and acupoint application was used most frequently.Inter-group comparison: There were significant differences in the distribution of acupuncture,moxibustion,auricular point therapy,acupoint application,ironing of traditional Chinese medicine,enema of traditional Chinesemedicine,rectal intake of drugs,and bloodletting therapy in each group.Cluster analysis and iteration classified the strongest combination of two external therapies:moxibustion and acupoint application.The research conclusion(1)Distribution of syndrome types from high to low is: Qi stagnation and blood stasis syndrome,cold coagulation and blood stasis syndrome,liver and kidney deficiency syndrome,Qi and blood weakness syndrome,dampness and heat stasis syndrome.(2)There are many kinds of prescriptions for primary dysmenorrhea treated by comprehensive treatment of traditional Chinese medicine,and many prescriptions are customized by clinicians.Different TCM external treatment schemes for primary dysmenorrhea can be referred to: ironing combined with acupoint application or moxibustion combined with acupoint application for qi stagnation and blood stasis syndrome;moxibustion combined with acupoint application for cold coagulation and blood stasis syndrome;moxibustion or traditional Chinese medicine ironing for liver and kidney deficiency syndrome;moxibustion for Qi and blood deficiency syndrome;acupoint application for dampness and heat stasis syndrome.
