Clinical Observation Of Jinshuijiaotai Decoction In The Treatment Of Chronic Pulmonary Heart Disease At Remission Stage (Deficiency Of Lung And Kidney Qi And Yin)

Objective: To observe the efficacy and safety of Jinshui Jiaotai Decoction in treating chronic pulmonary heart disease(CPHD)with deficiency of both lung and kidney Qi and Yin,and to explore its clinical value and popularization significance in treating CPHD.Methods: Sixty-four patients aged 45-80 who met the diagnostic criteria of chronic pulmonary heart disease in remission stage(deficiency of both lung and kidney qi and yin)were randomly divided into experimental group(32 cases)and control group(32 cases).In practice,one case was excluded in experimental group and two cases were excluded and dropped out in control group.The control group was given routine western medicine treatment(continuous oxygen therapy,budesonide formoterol powder inhalation(Xinbike Dubao)320/9.0 ug/inhalation,once in the morning and once in the evening;ambroxol hydrochloride oral liquid 30 ml oral TID treatment).On the basis of the treatment methods of the control group,the experimental group was treated with Jinshui Jiaotai Decoction,which was decocted by the pharmacy of traditional Chinese medicine in the hospital.Two months after treatment,the scores of TCM syndromes,CAT questionnaires,six-minute walking distance,blood gas analysis and the number of acute exacerbations after 6 months of follow-up were observed.Result:(1)There was no significant difference in baseline comparison of gender,age,course of disease,BMI,cardiac function classification,and number of acute exacerbations between the two groups(P > 0.05).(2)The total effective rate of the two groups: after treatment,the total effective rate of the main symptoms in the experimental group was 90.32%,and that in the control group was 66.67%.The difference was statistically significant(P < 0.05).In terms of symptom improvement,the total score of TCM symptoms in the two groups decreased after treatment(P < 0.01),and the experimental group was significantly better than the control group(P < 0.01)in terms of single symptoms,the experimental group and the control group.There was significant difference between the two groups(P < 0.05),but there was no significant difference between the control group and the control group in terms of lumbar and knee weakness,dizziness and tinnitus,hand,foot and heart warmth before and after treatment(P > 0.05).(3)After treatment,the CAT scores of both groups decreased(P < 0.01),and theexperimental group was better than the control group(P < 0.01).(4)After treatment,the six-minute walking distance increased in both groups(P <0.01).Compared with the control group,the experimental group was better than the control group(P < 0.01).(5)After treatment,the blood gas analysis values of the two groups were improved(P< 0.01),and the experimental group was better than the control group(P < 0.01).(6)Six months after treatment,the number of acute exacerbations in the two groups was significantly different(P < 0.05).The number of exacerbations in the experimental group was significantly less than that in the control group.(7)Safety evaluation: There were no significant abnormalities in blood routine,liver and kidney function,electrocardiogram and other related safety indicators during the treatment of the two groups.Conclusion: Jinshui Jiaotai Decoction has a good effect in improving symptoms,improving quality of life,improving heart function and reducing the number of exacerbations of acute attacks in the remission period of chronic pulmonary heart disease.It can also improve blood gas analysis index,and has reliable safety performance.It is worthy of clinical promotion.
