Analysis Of The Curative Effect Of Multiple Arthrodesis For Adult Talipes Cavus

Objective: To investigate the choice and curative effect of Multiple arthrodesis in the treatment of adult talipes cavus.Methods: This study used a retrospective analysis of cases,One hundred and forty-nine cases of talipes cavus underwent Multiple arthrodesis from September 2015 to May 2018 were retrospective analyzed;According to the manner of operations,68 patients were divided into A group;Triple Arthrodesis;45 patients were divided into B group:Ankle arthrodesis;36 patients were divided into C group:Pantalar arthrodesis.The AOFAS score,ICFSG score,Patient subjective satisfaction rating and VAS were used to evaluate the ankle joint function at preoperation and final follow-up.Preoperative and postoperative X-ray measurement results were compared.Results: postoperatively,140 patients were followed up,including 64 cases in group A for 12-31 months at an average of 20 months,48 of the 64 patients were male,and 26 were female.The average age at the time of surgery was 35(range,19-49)years;42 cases in group B for 15-31 months at an average of 25 months,22 of the 42 patients were male,and 20 were female.The average age at the time of surgery was 42(range,35-58)years;34 cases in group C for 13-28 months at an average of 22 months,18 of the 34 patients were male,and 16 were female.The average age at the time of surgery was 40(range,25-58)years.At the final follow-up,one patients had skin edge necrosis at the incision,but there were no symptoms of infection such as swelling and exudation.After dressing change,they healed.Two patients had nonunion at the osteotomy.One of them appeared one month after operation,one month after complete immobilization,the osteotomy healed.The other one appeared six months after operation and 6 months after shock wave treatment of ankle joint fusion.At the final follow-up,The AOFAS score of group A,B and C were:84.8±3.6?84.4±3.1?85.0±3.4;VAS score:2.4±0.5?2.5±0.5?2.5±0.5;ICFSG score:8.0±2.4?8.6±2.5?8.5±2.6.The differences were statistically significant(P<0.05).Patients with postoperative gait than preoperative significantly improved.The Meary angle,calcaneus tilt angle angle and Hibbs angle of group A,B and C were improved to some extent after operation.Patient subjective satisfaction rating of group A,B and C were:87.5%?92.8% ?91.2%.Conclusions:The application of Multiple arthrodesis in the treatment of talipes cavus can obtain excellent clinical efficacy,and it is a reliable bone operation for the treatment of talipes cavus deformity.
