Objective:This study aimed to analyze the dynamic changes of gastric polyp detection rates and Helicobacter pylori(Hp)infection rates over time,and to explore their potential influence factors.This study could provide scientific evidence for prevention of the occurrence of gastric polyps and infection of Hp.Methods:1.In this study,a total of 109,150 patients who underwent endoscopy examination at Qingdao Municipal Hospital from January 2007 to December 2017 were selected for consideration.4,515 patients with benign epithelial gastric polyps were enrolled,including 1,639 patients with multiple polyps and 2,876 patients with single polyps.The detection rate of gastric polyps at different time points and the composition of their pathological types were analyzed.The relationships of age,sex,and Hp infection with incidence of gastric polyps were analyzed.2.A total of 51,299 healthy individuals aged?18 year who underwent routine health examinations at Qingdao Municipal Hospital from April 2013 to June 2016 were enrolled.Urease-IgG antibody of Hp was detected using a ELISA kit to determine Hp infection.Data of average GDP and average real GDP were collected from the National Bureau of Statistics of China.The risk factors for Hp infection and dynamic changes of Hp infection rates over time were analyzed,and the association between economic status and Hp infection was explored.Results:1.The dynamic changes of gastric polyps detection rates:there was an increasing trend in the detection rates of gastric polyps(R~2=0.95),from 1.5%in 2007 to 7.1%in 2017.A similar trend was observed between the detection rate of multiple polyps and that of single polyps.This suggests that the incidence of gastric polyps is increasing year by year.2.Association between the age and detection rate of gastric polyps:there was an increasing trend in the endoscopic detection rate of gastric polyps had an increasing tendency with the age(R~2=0.99).It reached the peak incidence at 60-69 years old.There was no significant difference in the detection rate of gastric polyps between age?70 years and 60-69 years.Further analysis also showed a similar trend in the detection rate of multiple polyps and single polyps.These results suggest a positive association between the risk of gastric polyps and increased age.3.The detection rate of gastric polyps of different pathological types:the detection rate of gastric fundus polypoids and hyperplastic polyps increased year by year(R~2=0.95,R~2=0.86,respectively).The ratio of fundic gland polyp was increased gradually with time,meanwhile the ratio of hyperplastic polyps decreased.These results indicate that the risk of incidence of gastric fundus polyps is accelerating.4.The infection rate of Hp:The infection rate of Hp in patients with gastric polyps was 20.8%,significantly lower than that of the patients underwent endoscopy(32.3%,P<0.001).This suggests Hp infection may be have a negative association with the occurrence of gastric polyps.Moreover,the infection rate of Hp in patients with fundic gland polyps was significantly lower than that of patients underwent endoscopy(P<0.001).These results suggest a negative correlation between the risk of gastric polyps nd Hp infection.5.Analysis of risk factors for Hp infection:The overall prevalence of Hp in healthy individuals was 31.9%(16,382/51,299).Men,age,smoking,drinking,college education level below,rural place of residence,and average monthly income per household?3000 yuan are independent risk factors for Hp infection.6.Dynamic variation of the Hp prevalence in healthy individuals:The prevalence of Hp was highest in range of 1950-1959 birth cohort.This trend was observed in both male and female individuals.7.Correlation between economic status and the prevalence of Hp:With the increase of the er capita GDP and the actual per capita GDP of the country,the Hp infection rate shows a downward trend,and the power model was well fitted(R~2=0.91 and R~2=0.99,P<0.01 and P<0.001).This indicates that the increase of GDP is significantly ssociated with the decrease in the prevalence of Hp.The improvement in economic ituations may contribute to the reduction of the prevalence of Hp.Conclusions:1.The incidence of gastric polyps had a gradually increasing trend year by year,which was robably associated with the decrease in the infection rate of Hp and widely use of roton pump inhibitors.2.Men,age,smoking,drinking,college education level below,rural place of residence,and average monthly income per household?3000 yuan are independent risk factors for p infection.3.The infection of Hp presents a significant birth cohort effect,which may be related to he improved economic conditions.
