BackgroundThere are many studies on the effects of thyroxine on energy metabolism, protein, fat, carbohydrate and other substances in human body, but few studies on the effects of thyroxine on skeletal muscle and bone metabolism in human movement system. With the progress of science and technology, the process and relationship of these functional changes can be understood more accurately by laboratory examination, molecular diagnosis and analysis.Through the study on the influence of adult thyroxine level change on bone mass, bone density, muscle mass and the related relationship, the rule of human function change in the physiological process can be explained, which provides reference for the clinical formulation of measures for body composition change caused by abnormal thyroxine level.ObjectiveBone mass, bone density, muscle mass and other basic physical data of patients with abnormal thyroxine levels were investigated to explore the influence of thyroxine on bone mass, bone density and muscle mass. The correlation was analyzed by using statistics, the gender characteristics and physical fitness data under the action of different hormone levels were analyzed, and the internal laws and related factors of the differences were preliminarily discussed, which provided theoretical basis for the clinical risk assessment of muscle and bone and the prevention and treatment strategies.MethodsA total of 213 Patients in the department of endocrinology, the first affiliated hospital of Nanyang Medical College from January 1,2019 to February 29,2020 were selected as the subjects for the investigation.Conduct research under informed consent. The patient was more than 20 years old and clinically diagnosed as thyroid related disease. The basic information of height, body mass, muscle mass, bone mass and bone density were collected by using the experimental instruments, followed by the detection of serum thyroxine(FT4 and FT3),and the obtained data were statistically processed.According to whether the hormone level is abnormal, the patients were divided into low thyroxine group(A1 group),normal thyroxine group(B group)and high thyroxine group(A2 group).Results1.213 subjects were included in the survey, and there was no difference in age and height among the three groups.Group A1 had no difference in body mass and body mass index compared with group B,while A2 group was smaller than B group(P<0.05).2.Bone mass of the abnormal thyroxine group was lower than that of the normal group, especially A2 group(P<0.05).Bone mass had a strong internal relationship with T3 level, especially in women(P<0.01).There was no significant difference between bone mass and T4 level(P<0.05).3.The T-value and Z-value of bone mineral density in the abnormal thyroxine group were lower than that in the normal group(P<0.05),especially in the A2 group, presenting as osteoporosis. In A1 and A2 groups,T-value and Z-value were strongly correlated with thyroxine level(P<0.05).The bone mineral density of group B was strongly correlated with T3(P<0.05).4.The total body muscle mass, trunk muscle mass and lower limb muscle mass were statistically significant differences among the three groups(P<0.05).Group B was larger than group A1,while group A1 was larger than group A2.There was a strong negative correlation between total body muscle mass and trunk muscle mass and thyroxine(P< 0.01).The correlation between upper limb muscle mass and thyroxine level was not significant(P >0.05).5.Multivariate regression analysis showed that thyroxine levels were all included in the regression model of bone mineral density T-value,bone mineral density Z-value,bone mass, total body muscle mass and trunk muscle mass(P<0.05).Thyroxine level entered the upper limb muscle mass and lower limb muscle mass regression model in the female group(P > 0.05),but not in the male group.Conclusions1.Abnormal thyroxine level, especially the patients with high thyroxine group, had less bone mass, bone density and muscle mass than the normal group. There were gender differences in the effect of thyroxine on upper limb muscle mass.2.Thyroxine has multiple factors and mechanisms to affect and regulate bone and muscle metabolism in vivo, and T3 has a greater impact on bone and muscle metabolism than T4.3.This study selected bone mass, bone density and muscle mass index, most associated with the thyroid hormone level in their existence, the relationship between the mechanism of thyroid hormone may be in a variety of cells by changing the original balance of energy metabolism and change some pathways/medium/factor,which changes the cell physiological and pathological condition.
