Abstract/Summary:
Objective: Osteoporosis is one of the diseases that seriously affect the health andquality of life in middle-aged and elderly people which leads to a major cause ofdeath and disability.Therefore,it also makes great significances in earlyidentification and corresponding prevention.There are many predictors ofosteoporosis and fracture risks,a large-scale epidemiological study on theincidence of osteoporosis and fracture in China suggests that the currentpredictors may underestimate the risk of morbidity in Chinese populations.Ourresearch is based on the important effects of circulation,exercise and immunity onosteoporosis and we will also study whether the two can be used as predictors ofbone health in middle-aged and elderly people.Methods : Among the health care people who took part in the physicalexamination in our hospital from January 2015 to December 2016.Exclude relatedinterference factors,127 healthy people over 50 years old were selected.There are72 men and 55 women who are all the postmenopausal women.The carotid arteryof all subjects was examined by Italian esaote My Labsix + color Doppler ultrasounddiagnostic instrument.All of the subjects were divided into two groups according tothe presence or absence of atherosclerosis.Statistics show that the sex was matchwith age of the two groups.The subjects were divided into carotid arteriosclerosisgroup and non-carotid arteriosclerosis group according to the presence of carotidendarterectomy.The lumbar vertebrae(L1-L4)bone density of all subjects weremeasured by dual energy X-ray absorption method with the bone densitometer(Lunar Prodigy DF+352243,GE healthcare,USA).The average value of L1-L4 represented the bone mineral density of lumbar vertebrae.Meanwhile,the wholebody scanning and body composition analysis of the subjects were carried out byusing this bone densitometer and the ASM and SMI are calculated according torelevant formula.Separate analysis the correlation between carotid arteriosclerosis,SMI and lumbar bone mineral density((bone mineral density,BMD)to the bonemineral density of two-sided femoral neck and two-sided hip.Results:There exists differences(P<0.05)in the lumbar spine BMD ofarteriosclerosis group and the non-arteriosclerosis group,the left femoral neck(hereinafter referred to as the left femur)BMD,the right femoral neck(hereinafterreferred to as the right femur)BMD,the left hip joint(hereinafter referred to as theleft hip)BMD,and the absolute values of each index in the arteriosclerosis groupwere larger than those without arteriosclerosis.SMI was linearly associated withlumbar spine BMD,left femoral BMD,right femoral BMD,left hip BMD,and righthip BMD(P < 0.001).Multiple linear regression analysis was used to analyze theeffect of SMI and carotid atherosclerosis on bone mineral density,theconfounding factors such as gender and age was controlled。As a result,it showedthat the bone mineral density decreased after carotid arteriosclerosis.The level ofskeletal muscle index also had a significant effect on the bone mineral density of allsites detected(P<0.05).Conclusions : In summary,the bone density of lumbar spine,hip joint andbilateral femoral neck of patients with carotid atherosclerosis decreasedsignificantly compared with those without arteriosclerosis.The BMD of lumbarvertebrae was also increased in higher skeletal muscle index in healthypeople.Carotid arteriosclerosis and skeletal muscle index may be used as thepredictors of osteoporosis expect of sex hormone deficiency,the pain of the lowerback,low body weight,the short stature,the effects of drugs and history of brittlefracture,allof them may expand the prediction range of osteoporosis andassociated fractures.Carotid sclerosis and hypomyopathy can also be used ascontrollable factors of osteoporosis to intervene in advance and immediatelytreatment,they can make benelits for clinical application.
