Objective:To explore the effect of serum homocysteine levels on serum PON1 activity and carotid atherosclerosis.Methods:179 subjects from residents of health screening for cardiovascular disease were enrolled from October 2013 to July 2015 in some districts Shanghai.Recorded and gathered cardiovascular disease risk factor data,gave carotid artery ultrasound examinations and measured serum PON1 activity and the rest biochemical parameters of all subjects.Improved statistical processing to draw a conclusion.Research contents:(1)179 cases in the screening of risk factors of cardiovascular diseases(male >40 years old,women >50 years old or postmenopausal)from October 2013 to July 2015 in a community of Shanghai were enrolled in the study.The patients with history of acute coronary artery syndrome,chronic heart failure in acute exacerbation,acute heart failure,and malignant arrhythmia;cerebrovascular events occurred within2 months(transient cerebral ischemia,cerebral hemorrhage,cerebral infarction etc.);serious liver and kidney dysfunction;terminal malignant tumors,severe infections,severe pulmonary arterial hypertension and blood disease;long-term use of antioxidant drugs were excluded.(2)Detection indexes: completed determination of carotid artery ultrasound with Philip CX-50 by color Doppler ultrasound.Fasting peripheral venous blood of all subjects was drawn to determine of serum paraoxonase1(PON1)activity,fasting blood-glucose(FBG),glutamic-pyruvic transaminase(GPT),serum creatinine(Scr),blood uric acid(BUA),high-susceptible C reactive protein(Hs-CRP),homocysteine acid(HCY),total cholesterol(TC),triglyceride(TG),low density lipoprotein(LDL)and high density lipoprotein(HDL)etc.(3)Statistical methods: the data were analyzed with SPSS17.0.Data are represented as the mean ± standard deviation(SD).Student's t-test was used to compare between two groups.Correlation coefficient with spearman method and logistic regression independent risk factors analysis were used in correlation analysis.Differences were considered to be statistically significant at P values < 0.05.Results:1.Correlation analysis with Spearman method of serum HCY level and PON1 activity,r=-0.738,P < 0.01.2.According to serum homocysteine concentrations,all subjects were divided into two groups: high serum homocysteine level group(85 cases)and normal serum homocysteine level group(94 cases).Serum PON1 activity was significantly lower in high serum homocysteine level group than normal group(187.81±16.31 k U/L vs.288.58 ± 73.8k U/L,P < 0.01).And the difference of age,gender,systolic pressure,serum creatinine(Scr),blood uric acid(BUA),triglyceride(TG)and high density lipoprotein(HDL)were significant(P < 0.05).3.The incidence of carotid artery atherosclerosis was higher in group with up-regulated serum homocysteine than normal group(64.7% vs 44.7%,P < 0.05).4.All subjects were divided into PON1 activity down-regulated group and PON1 activity normal group.By single factor Logistics regression analysis,age,gender,serum creatinine(Scr),blood uric acid(BUA),triglyceride(TG),systolic pressure and serum homocysteine level influenced the activity of serum PON1.Multivariate stepwise Logistic regression analysis showed that increased serum HCY levels and blood creatinine were independent factors of serum PON1 activity.5.Chose carotid atherosclerosis occurs as dependent variables,parameters and serum HCY as independent variables to perform Separate Univariate and multivariate stepwise regression analysis respectively.Results showed that age and gender was independent factors of carotid atherosclerosis incidence and the down-regulates of serum homocysteine(HCY)level and serum PON1 activity were not independent factors of carotid atherosclerosis.Conclusions:1.Serum HCY and serum PON1 activity is negatively correlated.High serum HCY levels are independent factors affecting serum PON1 activity.2.The up-regulated serum HCY increases the incidence of carotid atherosclerosis,but down-regulated serum HCY level and serum PON1 activity are not independent factors of atherosclerosis.
