Correlation Between Fragile Intertrochanteric Fracture And Knee Joint Degeneration

Background: Low-energy hip fractures,which are represented by intertrochanteric fractures,are high-risk diseases in the elderly.The disability and mortality of these fractures are high.The existing literature shows that the brittle trochanter There are many studies on the pathogenesis of inter-fracture,and knee degeneration may be one of the risk factors for fragile intertrochanteric fracture.Objective: To analyze the clinical data and analyze the patients who were admitted to the hospital with a femoral intertrochanteric fracture due to mild violent trauma during the four years from March 2014 to March 2018 and treated with Gamma3 intramedullary nailing.Correlation between knee degeneration and brittle hip fracture provides early reference for the treatment of knee osteoarthritis,thus preventing the occurrence of hip fracture represented by intertrochanteric fracture.Methods: A retrospective study of 251 patients admitted to hospital for a single unilateral intertrochanteric fracture due to low-energy,mild violent trauma during the four years from March 2014 to March 2018.All patients included in the study were statistically analyzed for gender,age of onset,left and right sides of the disease,preoperative and postoperative VAS scores,length of hospital stay,and combined disease.All patients underwent routine radiographing of bilateral hip anterior and double The lateral knee joint was laterally sliced,and the bilateral knee joints were graded using the Kellgren and Lawernce scoring criteria,respectively,and the bilateral knee joint HHS scores were performed.The data were collected and integrated by Microsoft Office Excel 2003.The data were processed by SPSS 20.0 software.The correlation between the data was analyzed by chi-square test.The difference was statistically significant at P<0.05.Results and Conclusion: Among 251 patients with simple unilateral fragile intertrochanteric fractures,65 male patients(24 patients on the right and 41 patients on the left)accounted for 25.9%,and 186 female patients(93 on the right).93 cases)accounted for about 74.1%,and 2 tests were performed,2=3.31,P=0.069.The ratio of the left and right sides of the fractured limb was 1:0.87.The KL grading of the affected knee was: 0 grade 1 case;grade I 45 case;grade II 109 case III grade 95 case;grade IV 1 case;KL grade of the contralateral knee joint: 0 grade 2 case;grade I 123 For example,120 cases were grade II;6 cases were grade III;0 cases were grade IV;the average on the affected side of the KL grade was(2.20±0.09),and the average on the healthy side was(1.52±0.07);the difference between the two sides was statistically significant(p<0.05).The HHS score of the knee joint was(77.89±1.09)points on the affected side and(81.22±1.07)points on the healthy side.The difference between the two sides was statistically significant(p<0.05).The preoperative VAS score was(8.06±0.14)points,and the VAS score was(4.06±0.12)on the first day after surgery.The average hospital stay for251 patients was(14.51 ± 0.72)days,with the longest hospital stay being 48 days and the shortest being 5 days.There were 117 patients with unilateral fragile intertrochanteric fracture combined with coronary atherosclerotic heart disease and other cardiovascular diseases,87 patients with thyroid dysfunction and endocrine system diseases such as diabetes,including chronic bronchitis,asthma,etc.There were 48 patients with respiratory diseases,5 patients with digestive diseases such as gastric ulcer and atrophic gastritis,43 patients with cerebrovascular disease with stroke,and 12 patients with mental illness such as depression.There were 7 patients with Jinsen's disease,and some patients had multiple comorbidities.91 patients were involved.The above statistical results show that the degeneration of the knee joint has a certain promoting effect on the occurrence of low-energy intertrochanteric fractures.Surgical treatment can significantly alleviate the pain of patients,and has a positive effect on further rehabilitation.There was no significant difference(P>0.05).
