Background There are a large number of cardiovascular patients in China. And cardiovascular disease is the leading cause of death among urban and rural residents, which causes great burden in both health and economic areas.Cardiac rehabilitation can improve the quality of life of cardiovascular patients and their prognosis, which is recommended by class I for guidebook. However, the development of cardiac rehabilitation is confronted with many problems, such as low development rate, low utilization rate, poor compliance and so on.Patients are the direct recipients and beneficiaries of cardiac rehabilitation, who greatly decide the utilization rate and the effectiveness of cardiac rehabilitation programs.Howbeit, few surveys are currently conducted from the perspective of cardiovascular patients. Objectives Based on theory of planned behavior, the study is aimed at observing the willingness of coronary heart disease(CHD)patients to participate in cardiac rehabilitation so as to deeply analyze the internal and external influencing factors and to propose countermeasures which can promote the participation of coronary heart disease patients in cardiac rehabilitation. Methods Through literature review, this paper generally described cardiac rehabilitation and laid out fields for research.Guided by the theory of planned behavior, this paper made an interview outline and conducted semi-structured interview. On the basis of this, the research hypothesis is proposed.Designed through group discussion,a total of 280 questionnaires were distributed. Then the data was analyzed through SPSS20.0 statistical software.Results(1)The score of willingness to participate in cardiac rehabilitation in patients with coronary heart disease was 2.74±0.86.(2)Many factors had contributed to the significant difference in the willingness to participate in cardiac rehabilitation among CHD patients(p < 0.05),such as age, marital status, education level, monthly family income per capita, mode of medical expense bearing, difficulty in medical expense payment, physical activity, angina pectoris, dyspnea, treatment satisfaction, recreational activities, marital relationship.(3)Attitude,perceived behavior controland subjective norms were positively correlated with the willingness to participate in cardiac rehabilitation, accounting for 61.3% of the variance in willingness.(4)According to regression results, factors influencing the willingness to participate in cardiac rehabilitation included dyspnea, recreational activities, marital relationship, attitude, influence of family members and patients,as well as perceptual behavior control, which explained 65.1% of the variance in willingness. Conclusion Patients with coronary heart disease have a low willingness to participate in cardiac rehabilitation, which is greatly affected by their attitude, perceptual behavior control, family members and patients, physical conditions, and participation in recreational activities. It is suggested to attach greater importance to the development of cardiac rehabilitation and build a more supportive environment so as to improve heart rehabilitation motivation among medical institutions and staffs. Moreover,we should improve access to cardiac rehabilitation and strengthen cardiac rehabilitation education for CHD patients and their families, exploring the development model of cardiac rehabilitation which fits well with China's national conditions.
