Objective: To understand the current situation of wound ostomy and the practice of PICC specialized nurses in xinjiang,to understand the cognition and willingness of wound ostomy and PICC specialized nurses in multi-point practice in xinjiang,to provide Suggestions for the management and use of specialized nurses,and to lay a theoretical foundation for the implementation of multi-point practice policy for nurses.Methods: 1.Literature research method: to sort out and collect policy documents and literatures related to multi-point nurse practice.To understand the research status of multi-point nurse practice in China under the new situation of medical reform,and to explore the status quo of the cognition and willingness of multi-point nurse practice in PICC and wound stomatology specialty nurses in xinjiang.2.Cross-sectional survey method: a questionnaire survey was conducted among 304 specialized nurses of PICC and wound ostomy in xinjiang from March 2018 to June 2018 with the method of cluster sampling.Statistical methods: SPSS 17.0 statistical software was used to collate and analyze the data.One-way anova and multiple linear regression were used for analysis.AMOS21.0 was used for confirmatory analysis of the questionnaire.Results: 1.The cognitive score of multi-site practice of nurses in the specialty of PICC and wound ostomy was 32.85 7.807,which was in the middle or lower level;The willingness score was 40.97 12.073.Nurses in the specialty of PICC and wound ostomy have a low awareness of multi-point practice and a strong willingness to practice.2.The influencing factors of the cognitive scores of the specialized nurses in multi-site practice of PICC and wound stoma in xinjiang were working years,monthly income,grade and shift of specialized nurses;Factors influencing the multi-site practice of specialized nurses of PICC and wound ostomy in xinjiang were age,working years,monthly income,grade,shift and employment type of specialized nurses.3.The benefits of multi-site practice for specialized nurses of PICC and wound ostomy in xinjiang were successively listed as follows: 278(91.75%)believed that multi-site practice was conducive to giving full play to the professional expertise of nursing staff,maximizing the potential of nursing staff and realizing the self-value of nurses;257(84.81%)specialized nurses believed that multi-site practice could improve nurses' social status,enhance nurses' sense of professional identity and pride,and stimulate nurses' work enthusiasm.203(67.00%)specialist nurses believed that multi-site practice could reduce inpatient and unplanned readmission.The disadvantages were as follows: 185(61.05%)specialized nurses believed that multi-site practice would increase doctor-patient risk and affect nursing quality;182(60.07%)specialized nurses believed that multi-site practice would affect the medical order of the hospital and increase the difficulty of management.144(47.52%)specialist nurses believed that multi-site practice would cause brain drain of the first practice unit.4.Motivation and restrictive factors for nurses to carry out multi-point practice The survey found that 50(16.50%)specialized nurses chose to increase their economic income because of their motivation to carry out multi-site practice.128(42.24%)chose their own value reflected;76(25.08%)chose titles that were favorable to them.49(16.17%)chose to assist grassroots to improve service level;Among the factors restricting the promotion of multi-point practice of nurses,249(82.17%)specialized nurses chose relevant laws which were not perfect.214(70.63%)specialized nurses chose unclear medical risks and their interests could not be guaranteed.148(48.84%)specialized nurses chose the lack of perfect vocational information system;107(35.31%)specialized nurses chose their own jobs with heavy workload.The admission system for 176 nurses(58.09%)was not clear.159(52.48%)specialized nurses were lack of effective supervision.94(31.02%)specialist nurses chose their hospital for resistant behavior.5.The prospect of multi-site practice The results showed that 174 nurses(57.43%)were very optimistic about the prospects of multi-site nurse practice,and 46 nurses(15.48%)were optimistic.Seventy-two(23.76%)specialized nurses were not optimistic about the future of multi-point nurse practice.Eleven(3.63%)specialist nurses were not optimistic.Conclusion: 1.PICC and wound ostomy specialist nurses had low scores in multi-point practice cognition and strong intention of multi-point practice 2.age,highest educational background,professional title,employment type,and whether or not to carry out relevant nursing factors were the influencing factors of the multi-point practice intention of PICC and wound ostomy specialist nurses 3.under the premise of reasonable and standardized access system and legal protection,multi-site practice of PICC and wound stomatology specialist nurses can not only meet the needs of patients,but also optimize the allocation of human resources.
