Objective:To investigate the effect of body mass index(BMI)on acute respiratory distress syndrome(ARDS)in patients with acute pancreatitis(AP).Methods:A retrospective analysis of the incidence of abdominal pain in our hospital from 2014 to 2018,AP patients who were first diagnosed within 24 hours,were divided into two groups according to whether or not concurrent ARDS,and whether there was a difference in BMI between groups;in addition,definition and definition of obesity according to BMI values According to the BMI value of the patients at admission,they were divided into normal body group(18.5?BMI<25 kg/m~2),oveweight group(25?BMI<30 kg/m~2)and obese group(BMI?30 kg/m~2).The incidence of ARDS,the time of ARDS,and the severity of ARDS were compared between the groups.The ICU stay,total hospital stay,and 28-day mortality were compared between AP patients in different BMI groups.Results:1)The BMI value of patients with AP complicated with ARDS was significantly higher than that of patients without ARDS.2)The incidence of ARDS in patients with oveweight group and obesity was significantly higher than that of normal body group,and the incidence of ARDS in obese patients was also significantly higher than that in oveweight group.3)The incidence of ARDS in the obese group was earlier than that in the oveweight group and normal body group,but there was no significant difference in the time of onset of oveweight group and normal group ARDS;4)the lengh of stay of ICU and total hospitalization time in the obese group were higher than the oveweight group and normal body was significantly prolonged,but there was no significant difference in28-day mortality between the three groups.Conclusion:The high BMI value is a risk factor for ARDS in AP.The higher the BMI value,the more likely AP may be concurrent with ARDS.Therefore,the simple and easy calculation of BMI can be used in clinical work to alert us to more closely monitor the changes in the condition of overweight or obese patients,and to seek valuable treatment time for preventing and reducing organ failure in patients with acute pancreatitis.
