A Clinical Research On Patients Of Phlegm-dampness Syndrome Of Polycystic Ovary Syndrome In Different Glucose Metabolism States

Objective:Through the analysis and comparison of clinical features and biochemical indicators indices data got from the patients phlegm-dampness syndrome of PCOS in different glucose metabolism states,and discuss the correlation between glucose metabolism status and clinical indicators in patients with phlegm-dampness syndrome of PCOS,to provide basis for clinical diagnosis and treatment of phlegm-dampness syndrome of PCOS with abnormal glycometabolism and prevention of long-term complications.Methods: 952 patients of phlegm-dampness PCOS in the First Affiliated Hospital of Heilongjiang University of Chinese Medicine from January 2015 to January 2019 were selected.According to the classification criteria of glucose metabolism,the patients were divided into five groups: normal glucose test(NGT)group,impaired fasting glucose(IFG)group,impaired glucose tolerance(IGT)group,impaired glucose homeostasis(IGH)group and type 2 diabetes mellitus(T2DM)group.Clinical characteristics,reproductive endocrine hormones and lipid metabolism indexes of each group were collected for statistical comparison and analysis.Results:1.The incidence of abnormal glucose metabolism in patients with PCOS was33.1%(315/952),of which IFG group accounted for 2.3%(22/952),IGT group accounted for22.0%(209/952),IGH group accounted for 2.8%(27/952),T2 DM.Group accounted for6.0%(57/952).2.Compared with the NGT group,BMI and WHR levels of the 4 groups with abnormal glucose metabolism increased,and the difference was statistically significant.SHBG levels in reproductive endocrine hormones decreased with the increase of abnormal glucose metabolism,and the difference was statistically significant.There were no significant differences among FSH,LH,LH/FSH,T and DHEAS groups.3.Whit the aggravation glucose metabolism,the glucose and insulin levels gradually increased.Insulin peaks in the NGT group,the IFG group and the IGH group occurred 1hour,and the IGT group and the T2 DM group where insulin peaks occurred at 2 hours.4.The incidence of dyslipidemia in each group of patients with sputum and wet PCOS increased with the progression of abnormal glucose metabolism,with the highest incidencein the T2 DM group.Compared with NGT group,the Blood lipid index TG,TC,LDL-C and Apo-B Water of IGT Group,IGH Group and T2 DM group increased on average,while HDL-C level decreased,while the difference of IFG group performance was not significant.There was no statistically significant difference between the Apo-A groups.5.BMI,WHR,AUC-G,AUC-I,TG,TC,LDL-C,ApoB/ApoA,ApoB and FPG,G120,HOMA-IR were positively correlated,SHBG,IAI,HDL-C and FPG,G120,HOMA-IR were negatively correlated,AUC-I/AUC-G is positively correlated with G120 and HOMA-IR.AUC-I/AUC-G is positively correlated with G120 and HOMA-IR.I30/G30 was negatively correlated with FPG and G120,and I30/G30,ApoA and HOMA-IR were positively correlated,and the difference was statistically significant.Conclusion:1.The patients of phlegm-dampness PCOS had a greater incidence hyperglycemia.OGTT and OGIRT are necessary for screening hyperglycemia in patients of phlegm-dampness PCOS.2.The Glucose metabolism is associated with the level of SHBG and is not related to androgen levels.3.The increase degree of impaired glucose metabolism was correlated with lipid metabolic disorders.
