At present,the overall medical resources in China are unevenly distributed.The tertiary hospitals are overcrowded and the basic hospitals are deserted.The medical treatment combination will help adjust and optimize the layout of medical resources,improve the overall effectiveness of the medical service system,and better implement hierarchical medical treatment and meeting the needs of the people.The establishment of a scientific and rational medical facility layout is the primary task of the development of medical treatment combinat io n and it is also an important measure to relieve the pressure of medical resources and improve the efficiency of medical resources utilization.This article takes the tertiary hospitals and community hospitals in a certain area as the research object,and studies the network design problems of the medical treatment combination under the hierarchical medical system from the government's perspective.The article selects some hospitals from the candidates of the tertiary hospitals and community hospitals as members of the medical treatment combination and establishes hierarchical links between the tertiary hospitals and the community hospitals so that to maximize the number of patients can be covered and minimize the cost of constructing the medical treatment combination.The main research content is divided into the following two parts:(1)Assuming that the patient's condition is the same.First of al,through the analysis of the patient's choice of medical behavior,the medical technology level and the expected total time of consumption are the two major factors affecting the patient's medical treatment choice.On this basis,the negative utility function of the patient's medical treatment is established,and then the patient's hospital choice model is established.Finally,on the basis of the maximum coverage model,the patient's hospital choice model was taken as a constraint,and the two-layer hospital service network planning model was established.Numerical experiments show that: There is a positive correlation between the cost of construction and the number of people covered by the medical treatment combination.The government should correctly weigh the relationship between the number of covered patient and the total cost;the patient's sensitivity to the total expected time consumption ? has a negative correlation with the total number of people seeking medical treatment.Full consideration of the patient's medical preferences al ows the designed medical network to better meet the needs of the patient.(2)Assuming that the patient's condition is different,the patient is divided into two categories: emergency patients and non-emergency patients.Firstly,the medical choice behaviors of patients with different disease conditions were analyzed and it was concluded that patients have different preference to medical treatment in different disease conditions.On this basis,the negative utility function of the two types of patients is established,and the emergency patients have priority in seeking medical treatment and a hospital choice model for two types of patients is established.Finally,on the basis of the maximum coverage model,the two types of patients hospital choice model was taken as a constraint,and a two-layer hospital service network planning model was established.Through numerical analysis found: The proportion of emergency patients in tertiary hospitals is relatively higher,while the proportion of nonemergency patients seeking medical care in primary hospitals is higher,so patients with different disease conditions should be guided to seek medical care.(3)Taking the medical treatment combination in Gulou District of Nanjing City as the research object,redesign the network of its medical treatment combination.The redesigned network changed the pattern of a primary hospital attached to a tertiary hospital,increased the connection between hospitals at different levels,and increased the number of patients.On the one hand,the research of this paper provides novel ideas for the network design of medical treatment combination,and provides reference values for decision makers such as governments.On the other hand,it is of practical significance to guide patients to seek medical care and form a fair and well-organized medical environment.
