Effects Of Nursing Intervention Based On Protection Motivation Theory On The Knowledge, Behavior And Biochemical Indicators Of Self-monitoring Of Blood Glucose For Patients With Diabetes Mellitus

Objective: To explore the influence of the nursing intervention based on protection motivation theory on knowledge, behavior and biochemical indicators of self-monitoring of blood glucose for patients with diabetes mellitus, analysis the effect of the nursing intervention and provide new methods of intervention for self-monitoring of blood glucose.Methods: 1 Subjects The hospitalized patients diagnosed with type 2 diabetes in endocrinology department of A hospital. 2 Research methods Screening the patients hospitalized in endocrinology department in January to March 2014, 100 diabetic patients were chosen by random sampling method for the study. Patients were divided into the intervention group and the control group with 50 in each. Patients in control group were received general care and patients in intervention group were given a further 6-month protection motivation theory-based nursing intervention. According to the speciality care requirements, patients in control group received diabetic education 2 times per week during their hospitalization, 30-45 minutes per time. The contents mainly include diet therapy, therapeutic exercise, blood glucose self-monitoring. For the patients in the intervention group, 7 factors of the patient protection motivation(intrinsic rewards, external rewards, exposure, susceptibility, response efficacy, self-efficacy and response costs) are analysised at first. According to the assessment results, for the purpose of improving the awareness of the severity and susceptibility of diabetescomplications, improving self-efficacy, reducing intrinsic and external rewards rewards and effectively promoting the formation of protective motivation, a 6-month protection motivation theory-based nursing intervention is proposed. Main contents of the nursing intervention include health education, demonstration operation, complications of diabetes experiential education. "glucose monitoring records" and "Diabetes Handbook" are issued to the patients before discharge, regular follow-up and individualized instruction are proposed after discharge from hospital. In order to evaluate the intervention effectiveness between the two groups, the influence of the knowledge and skill of self-monitoring of blood glucose, protection motivation of self-monitoring of blood glucose,behavior,blood glucose, glycosylated hemoglobin were evaluated respectively before the intervention, after 3 months and 6 months of intervention. 3 Statistical analysis All data were analyzed with statistical software SPSS 17.0. Two sample t-test after normality test is proposed to compare the measurement data and the comparing of the data are represented by X ± S. Repeated measurement data is analyed by the variance of repeated measurement design data. Count data is analyed by fourfold table chi-square test or R × C table chi-square test. for the data less than 50% of the theoretical(fourfold table) or data is less than 33%(R × C table), Fisher’s exact test is used, t test and Chi-square tests in this study use two-sided test, test level a = 0.05.Results: 1 96 patients have completed the entire process, including 48 patients in the intervention group and 48 in the control group. 2 There was no statistically significant difference in demographic characteristics between the two groups, such as gender, age, occupational status, education, health care approach, economic status and disease cases(P> 0.05). 3 There was no difference in knowledge of blood glucose and blood glucose monitor operation between the two groups before the intervention.After the intervention, the score in intervention group was higher than that of the control group, which means that there was a significant difference between two groups(P < 0.05). 4 There was no difference in protection motivation of self-monitoring of blood glucose between the two groups before the intervention. After the intervention, the situation in intervention group was better than that of the control group, which means that there was a significant difference between two groups(P < 0.05). 5 There was no difference in frequency of monitoring of blood glucose and glycosylated hemoglobin between the two groups before the intervention. After the intervention, the frequency in intervention group was higher than that of the control group, which means that there was a significant difference between two groups(P < 0.05). 6 There was no difference in blood glucose and glycosylated hemoglobin between the two groups before the intervention. After the intervention, the results in intervention group was better than that of the control group, which means that there was a significant difference between two groups(P < 0.05).Conclusions: 1 Nursing interventions based on protection motivation theory can promote to grasp knowledge of blood glucose and blood glucose monitor operation. 2 Diabetic patients are lack of protection motivation, nursing interventions based on protection motivation theory can promote protection motivation and help patients establish regular behaviors of self-monitoring of blood glucose. 3 Diabetic patients’ frequency of monitoring of blood glucose and glycosylated hemoglobin is less than the standard. Nursing interventions based on protection motivation theory can promote the frequency of monitoring of blood glucose and glycosylated hemoglobin. 4 The blood glucose and glycosylated hemoglobin of diabetic patients can get standard rarely, Nursing interventions based on protection motivationtheory can promote diabetic patients’ blood glucose and glycosylated hemoglobin up to the standard level.
