Objetive:Abstracting the topic of self-management needs of patients with Rheumatoid arthritis(Rheumatoid arthritis,RA)through interviews,formulating nursing intervention programs based on Protective motivation theory,observing the effects of nursing intervention on self-management ability,quality of life and physiological indicators in patients with Rheumatoid arthritis,and providing reference for nursing management in patients with Rheumatoid arthritis.Method:1.From January to April 2019,the purpose sampling method was used to conduct a semi-structured interview with 10 patients with Rheumatoid arthritis to obtain their self-management needs theme.2.From May to October 2019,70 patients with Rheumatoid arthritis in a Grade 3A hospital in Shanxi province who met the admission criteria were selected as control group in May-July 2019 according to the order of admission,and 35 patients in each group in August-October 2019.routine nursing interventions were implemented in the control group.nursing interventions based on protective motivation theory were implemented in the intervention group.chronic disease self-management behavior scale,arthritis self-efficacy scale-8(ASES-8),RA quality of life scale,inflammatory indicators(erythrocyte sedimentation rate,C reaction protein),DAS28 score,and so on to evaluate the effect.Result:1.Abstracted 4 Rheumatoid arthritis self-management needs topics :(1)changing self-care(2)mental accommodation(3)common decision-making by nurses(4)social and peer support.2.Comparison between the two groups before intervention:(1)There was no significant difference between the two groups in terms of age,sex,course of disease,occupation and so on(P>0.05).There was no significant difference between the two groups in terms of CDSMS score,ASES-8 score,RA quality of life scale score,inflammatory indicators(Erythrocyte sedimentation rate,C reaction protein),DAS28 score,and so on(P>0.05).(2)Post-intervention comparison between groups: After the intervention,the CDSMS score of the intervention group was higher than that of the control group(P<0.05).After the intervention,the ASES-8 score of the intervention group was higher than that of the control group,which was statistically significant(P<0.05);There was no significant difference in quality of life scale score between the two groups after intervention(P>0.05).There was no significant difference in ESR between the two groups after intervention(P>0.05).There was significant difference in CRP and DAS28 score between the two groups after intervention(P<0.05).(3)After the intervention,the CDSMS scores of the intervention group were significantly higher than those before the intervention,the difference was statistically significant(P<0.05),and the CDSMS score of the control group was not significantly different from that before the intervention(P>0.05);the ASES-8 score of the intervention group was significantly higher than that before the intervention,the difference was statistically significant(P<0.05),and the ASES-8 score of the control group was higher than that before the intervention.The difference was little statistically significant(P>0.05).The quality of life of the two groups decreased compared with that before the intervention,the difference was statistically significant(P<0.05);the inflammatory indicators(Erythrocyte sedimentation rate,C reaction protein),DAS28 score of the intervention group decreased compared with the intervention,the difference was statistically significant(P<0.05),the inflammatory indicators(Erythrocyte sedimentation rate,C reaction protein),DAS28 score of the control group The difference was statistically significant(P<0.05).Conclusion:1.Rheumatoid arthritis patients have 4 self-management needs topics :(1)changing self-care(2)mental accommodation(3)common decision-making by nurses(4)social and peer support.2.Nursing intervention based on Protective motivation theory can improve the self-management ability,self-efficacy and quality of life of patients,which is worthy of clinical practice promotion.
