Objective:To observe the changes of Vaspin and IL-1 serum levels in patients with type 2diabetes mellitus(T2DM)and type 2 diabetic kidney disease(DKD)and explore their relationship with DKD.Methods:We enrolled 71 T2 DM patients in our hospital.According to the urinary albumin to creatinine ratio(ACR),the patients were divided into two groups: simple diabetes mellitus(SDM)group(ACR ? 30mg/g)and DKD group(ACR>30mg/g).We also enrolled 32 healthy controls from the hospital in the same period as control group.Serum Vaspin and IL-1 levels were detected by ELISA,and their relationship with DKD were analyzed by Logistic regression analysis.Results:1.There was no significant difference between the three groups in gender and age(P>0.05),indicating that the three groups were comparable.BMI of SDM group and DKD group was higher than that of NC group(P<0.05).SBP of DKD group was higher than that of NC group and SDM group(P<0.05),but there was no significant difference in DBP among the three groups.In terms of biochemical indexes,FBG,FIns,HOMA-IR,HbA1 c and TG of the SDM group and DKD group were all higher than those of the NC group(P<0.05).There was no significant difference in TC,LDL-C and HDL-C between the three groups(P>0.05).Serum Vaspin levels in DKD group were significantly lower than those in SDM group and NC group(P<0.01),but their IL-1 levels were higher thanthose in the other two groups(P<0.01).Serum Vaspin and IL-1 levels in SDM group were significantly higher than those in NC group(P<0.01).2.In T2 DM patients(including SDM and DKD groups),univariate correlation analysis showed that serum Vaspin level was positively correlated with BMI and HDL-C,and significantly negatively correlated with IL-1 and disease course.Correlation analysis of serum IL-1 with other clinical indicators showed that only Vaspin showed a significant negative correlation with IL-1.Taking serum Vaspin level as dependent variable,age,gender,FBG,FIns,HOMA-IR,HbA1 c,TG,TC,LDL-C,HDL-C,BMI,IL-1 and course of disease as independent variables,multivariate stepwise regression analysis was performed,suggesting that BMI,IL-1,and HDL-C were independent risk factors for Vaspin level.3.With or without DKD as the dependent variable,gender,age,course of disease,BMI,SBP,DBP,FBG,FIns,HOMA-IR,HbA1 c,TG,TC,LDL-C,HDL-C,IL-1,and Vaspin as the independent variables for Logistic regression analysis,the results showed that Vaspin,IL-1 and course of disease entered the equation.Conclusion:Serum Vaspin was closely related to IL-1 level,BMI,course of disease and HDL-C in T2 DM patients.Vaspin may play a protective role in the occurrence and development of DKD,while long disease duration and high IL-1 may be important inducers of DKD.
