[Object]To understand the cognitive differences,attribution differences and influencing factors of doctors and patients pertaining to trends in doctor-patient relationship,to identify the causes of cognitive differences between doctors and patients and to make policy recommendations for further targeted healthcare improvement.[Methodl The subjects of this study are inpatients and doctors sampled from 136 tertiary hospitals in China.Based on the sociodemographic information and doctor-patient relationship evaluation collected by questionnaire,SPSS22.0 was used to conduct Mann-Whitney test.Pearson's chi-squared test and multivariate logistic regression analysis to compare and analysis cognitive differences,attribution differences and influencing factors of doctors and patients pertaining to trends in doctor-patient relationship.[Results]There was a cognitive difference in trends in doctor-patient relationship between doctors and patients was statistically significant(P<0.05).85.63%of patients agreed that the doctor-patient relationship improved in the past two years,while the result in doctors was 33.19%.Doctors attributed the tension between doctors and patients to external factors,but patients supported it caused by medical factors(P<0.05).The regression results showed that fixed doctors,shared decision-making and human care from doctors(P<0.05)could improve the patient's recognition of the trends in doctor-patient relationship,adequate doctor-patient communication,acceptable workload and medical liability insurance(P<0.05)could improve the doctor's recognition.[Conclusions]Doctors are less optimistic about the trends in doctor-patient relationship than patients,and there is attribution difference between doctors and patients about the tension in doctor-patient relationship.Therefore,it is recommended that doctors and patients should improve their understanding through two-way enpathy.The family doctors' contracting system and medical associations in different-level hospitals could be prompted to ease the pressure of doctors in tertiary hospitals and increase the trust of doctors.Legislative construction and the positive role of the media also should be emphasized to optimize the social environment.The establishment of a mandatory system of medical liability insurance is necessary to protect the legitimate rights and interests of both doctors and patients.
