Study On The Evaluation Of The Doctor-patient Relationship And Its Influencing Factors In Hunan Province Tumor Hospital

Objective:To study the doctor-patient relationship and its influencing factors, and help hospitals improve the doctor-patient relationship, improve service quality and enhance competitiveness. It also provides a reference for building a harmonious doctor-patient relationship, and has a positive role in promoting the hospitals’ development and maintaining social harmony and stability.Methods:Sampling in two stages and starting with the simple random sampling, questionnaire survey was randomly selected from34clinical departments of a third-grade class-A hospital, drawing to9sections. Each Department were delivered35questionnaires. Then in accordance with the conditions, inpatients from each department were random sampled, until the department completed35questionnaires. The contents of the investigation included the tumor inpatients1age, gender, region, marital status, religious belief, education level, occupation, residence, income level and other social demographic characteristics such as social factors, inpatients’satisfaction on impact of the evaluation of doctor-patient relationship, and so on.Results:A total of315questionnaires were delivered. After excluding19invalid questionnaires, the remaining valid questionnaires are301and the effective recovery rate was95.6%.Among301inpatients surveyed,38.2%of the patients thought the doctor-patient relationship was very harmonious,36.9%of the patients thought the doctor-patient relationship was harmonious, and16.6%of the patients thought the doctor-patient relationship was general,5.3%of the patients thought the doctor-patient relationship was a little nervous, and3.0%of the patients thought the doctor-patient relationship was very nervous. The judgment was mainly based on their own personal feelings (83.1%).Nearly half (49.5%) of the patients thought a satisfactory medical service of acquaintances was related to a relationship in the hospital.69.7%of the patients thought medical staff was in good condition or very rich. The majority of patients (74.8%) fully trusted in doctors when go to see doctors while25.2%were suspicious of doctors.80.1%of patients or their family members didn’t have a red envelope behavior and the reason for those who gave a red envelope ranked first was that they thought the doctor would be more responsible. Most of the patients and their family members in the process of medical treatment did not happen the medical dispute (94%). And if the medical disputes happened, the most preferred way to handle the issues were to consult with the doctor party (38.2%), through legal means (37.9%), to hospitals or units (37.2%), and1.3%patients might consider using violent means to solve the issue.Only about half of the patients had confidence in the judicial authentication (56.2%), and the very confident group was only16.3%. Nearly half of the patients thought the reports of media to medical disputes were credible (46.2%), a significant portion of patients with neutral (41.5%). Nearly half of the patients thought the media played a positive role in the doctor-patient relationship (43.2%). When seeing to a doctor, patients were not satisfied with the previous ordinal because of the high cost (76.4%), attendance procedures complex (62.5%) and bad attitude of health care personnel (18.3%). On the issue of attribution to the doctor-patient relationship,48.8%of patients thought that the Government should bear the main responsibility,44.5%of patients believed that hospitals and doctors should take the primary responsibility,4.3%of patients believed that patients should bear the main responsibility.Most of the patients were optimistic about the improvement of the doctor-patient relationship in the future (87.4%) while12.6%of the patients keeped an pessimistic attitude. The patients considered that initiatives to promote the development of a good doctor-patient relationship rowed of the top3were in the following order:a. improve the quality and medical level of medical staff (67.40%); b. the government strengthen the health input (64.80%); C. to establish and improve the doctor-patient communication channels (62.50%).Multiple factors analysis showed that the influencing factors of evaluation of doctor-patient relationship were education, income, source of judgment, expectations to see a doctor, the media credibility, when referring to the confidence in the doctors and patients satisfaction with the medical service.Conclusion:75.1%of the patients think that the doctor-patient^relationship is harmonious,87.4%of the patients are optimistic about the improvement of doctor-patient relationship in the fure. The influencing factors of evaluation of doctor-patient relationship are education, income, source of judgment, expectations to see a doctor, the media credibility, the confidence in the doctors and patients satisfaction with the medical service. The government and relevant departments of the hospital should continue to take the measures to improve the doctor-patient relationship, and further improve the doctor-patient trust degree between doctors and patients, reducing medical disputes and the medical trouble in order to the formation of a good medical order and social environment and the harmony and stability of our society.
