Objective:To observe the effects of the classical famous Guilingji on the fatigue degree,activity ability,lung function,clinical symptoms and mental state of male patients with stable kidney-deficiency and chronic obstructive pulmonary disease,and to provide new ideas for the application of Guilingji in chronic diseases.Research methods:1 Research designThirty male patients with stable chronic obstructive pulmonary disease(COPD)of deficiency of kidney essence were selected from September 2018 to December 2018 in the respiratory department of Guang'anmen Hospital,Chinese Academy of Traditional Chinese Medicine.They were randomly divided into experimental group and control group with 15 cases in each group.Both groups received routine western medicine treatment and pulmonary rehabilitation training.The experimental group took Guilingji medicine,the control group took Guilingji starch simulator,and Guilingji or simulator took two capsules(0.3g/granule)20 minutes before breakfast.Pulmonary rehabilitation training is conducted twice a week for 90 minutes each time.The pulmonary rehabilitation training content follows the pulmonary rehabilitation program based on the Baduanjin,Wuqinxi,six-character pithy,and Taijiquan.Fatigue was assessed using the Fatigue Scale FS-14 questionnaire,which included physical fatigue scores and mental fatigue scores;activity ability using a six-minute walk test(6MWT),exercise cardiopulmonary index(Loadmax,VO2max,VO2max/kg,VO2max/expected VO2max)was evaluated;clinical symptoms were assessed using the Dyspnea Index(mMRC)questionnaire and the COPD Assessment Test(CAT)questionnaire;mental status was assessed using the Self-rating Anxiety Scale(SAS)and the Self-rating Depression Scale(SDS).Using SPSS22.0 software to statistically analyze the fatigue score,body fatigue score,mental fatigue score,six-minute walking distance(6MWD),exercise cardiopulmonary index(Loadmax,VO2max,VO2max/kg,VO2max/expected VO2max),lung function indicators(FEVi,FVC,FEVi/FVC),mMRC score,CAT score,SAS score,SDS score before and after the experiment.2 Course of treatmentThe experimental observation period was 3 months.3 Observation indicatorsThe main efficacy indicators included fatigue score(physical fatigue score,mental fatigue score),and six-minute walking distance(6MWD).Secondary efficacy measures include cardiopulmonary exercise tests(Loadmax,VO2max,VO2max/kg,VO2max/expected VO2max),pulmonary function indicators(FEV1,FVC,FEV1/FVC),clinical symptoms(mMRC score,CAT score),mental status(SAS score,SDS score).Results:1.After the experiment,the fatigue score and body fatigue score of the two groups were significantly improved compared with those before the experiment,with statistical significance(P<0.05);there was statistical significance between the two groups(P<0.05).The scores of mental fatigue in both groups were lower than those before the experiment(P>0.05);there was no significant difference between the two groups(P>0.05).2.After the experiment,the six-minute walking distance of the two groups was significantly improved compared with that before the experiment(P<0.05),and there was statistical significance between the two groups(P<0.05).3.After the experiment,the indexes of exercise cardiopulmonary Loadmax,VO2max,VO2max/kg,VO2max/predicted VO2max in the two groups were not significantly improved compared with those before the experiment(P>0.05);there was no significant difference between the two groups(P>0.05).4.After the experiment,the lung function indexes FEV1,FVC1FEV1/FVC of the two groups did not improve significantly(P>0.05),but there was a delayed downward trend;there was no significant difference between the two groups(P>0.05).5.After the experiment,the mMRC score and CAT score of the two groups decreased compared with those before the experiment(P>0.05).The improvement of mMRC score and CAT score of the experimental group was better than that of the control group,but there was no significant difference between the two groups(P>0.05).6.After the experiment,the SAS and SDS scores of the two groups were lower than those before the experiment(P>0.05),and there was no significant difference between the two groups(P>0.05).Conclusions:1.Guilingji may be helpful to alleviate the physical fatigue of male patients with chronic obstructive pulmonary disease of kidney essence deficiency in stable stage.2.Guilingji may be helpful to improve the activity of male patients with chronic obstructive pulmonary disease of kidney essence deficiency in stable stage.
