Clinical Characteristics Of Ocular Accommodation In Juvenile Progressive Myopia

Objective:To analyze and discuss the clinical characteristics of the ocular accommodation parameters related to progressive myopia,so as to provide reference basis for clinical research on prevention and control of juvenile myopia.Method:The subjects of this study were 26 adolescents with emmetropia and 112 adolescents with myopia(including 43 non-progressive myopia and 69 progressive myopia according to the progression of myopia).All subjects were examined objectively by compound tropicamide mydriasis on the basis of computer auto-optometry,and then re-examined under the comprehensive optometry.After accurate binocular diopter,eye accommodation parameters(positive relative accommodation,negative relative accommodation,accommodation response,amplitude of accommadation,accommodation facility)were measured on a comprehensive optometry instrument,and the results of relevant accommodation parameters among groups were statistically analyzed.Results:1.There was no significant difference in sex ratio and age among the progressive myopia group,non-progressive myopia group and emmetropia group,p > 0.05.2.Accommodation ResponseIn the emmetropic group,the proportions of lag,equivalence and lead were 19.23%,42.31% and 38.46% respectively.The proportion of accommodation lag,accommodation equivalence and accommodation lead in progressive myopia group were 53.62%,30.43% and 15.95% respectively.The proportion of accommodation lag,accommodation equivalence and accommodation lead in non-progressive myopia group were 44.19%,34.88% and 20.93% respectively.There were significant differences in the composition ratio of accommodation response types among the three groups(P < 0.05).There was no statistical difference in the composition ratio of accommodation response types between different refractive degrees in progressive myopia group(P > 0.05).3.Positive Relative AccommodationThe positive relative accommodation mean values of emmetropia group,progressive myopia group and non-progressive myopia group are-(3.30± 0.92),-(2.24 ±0.97),-(2.73±0.91),respectively.Compared with emmetropia group,positive relative accommodation(PRA)of progressive myopia group and non-progressive myopia group are significantly reduced(P < 0.05),while positive relative accommodation of progressive myopia group is significantly lower than that of non-progressive myopia group(P < 0.05).There was no significant difference in positive relative accommodation between different refractive degrees in progressive myopia group(P > 0.05).4.Negative Relative RegulationThe negative relative accommodation mean values of emmetropia group,progressive myopia group and non-progressive myopia group were(2.31±0.20),(2.19±0.34),(2.18±0.30),respectively.There was no significant difference in negative relative accommodation among emmetropia group,progressive myopia group and non-progressive myopia group(P > 0.05).There was no significant difference in negative relative accommodation between different refractive degrees in progressive myopia group(P > 0.05).5.Amplitude of AccommadationThe mean amplitude of accommadations of emmetropia group,progressive myopia group and non-progressive myopia group were(12.21±1.02),(10.54±1.7)(11.45±1.42),respectively.compared with emmetropia group,the amplitude of accommadations of progressive myopia group and non-progressive myopia group were significantly reduced(p < 0.05).The accommodation range of progressive myopia group was significantly lower than that of non-progressive myopia group(P < 0.05).The amplitude of accommadation between different refractive degrees in progressive myopia group was compared in pairs.the amplitude of accommadation in high myopia group and moderate myopia group was significantly lower than that in mild myopia group(p < 0.05).there was no significant difference between moderate myopia group and high myopia group(p > 0.05).6.Accommodation FacilityThe mean accommodation facility of emmetropia group,progressive myopia group and non-progressive myopia group were(7.46±0.99),(6.01±1.04),(7.12±1.16),respectively.the accommodation facility of progressive myopia group was significantly lower than that of non-progressive myopia group and emmetropia group(p < 0.05).there was no significant difference between emmetropia group and non-progressive myopia group(p > 0.05).There was no significant difference in accommodation sensitivity between different refractive degrees in progressive myopia group(P > 0.05).Conclusion : 1.Compared with emmetropia and juvenile non-progressive myopia patients,juvenile progressive myopia patients have a higher accommodation lag ratio,and the positive relative accommodation,accommodation facility and amplitude of accommadation are significantly decreased.2.Juvenile progressive myopia is relatively insufficient accommodation function.
