| ObjectiveThe main purpose of this project is to analyze the clinical observation indexes of three different incision treatment methods in the annulus of lumbar disc surgery through clinical observation,to find a reasonable and effective treatment method for the treatment of the annulus,thus improving the traditional operation and making the tradition Classical surgery has a low recurrence and more stable effect.MethodsThe study used a randomized group experiment to analyze the exclusion criteria for patients undergoing lumbar disc surgery from October 2016 to December 2017 due to lumbar disc herniation to Zhuhai Hospital of Guangdong Provincial Hospital of Traditional Chinese Medicine.Randomly assigned to the suture group,the annular incision group,and the inline incision group according to a ratio of 1:3:3.The patient’s gender,age,follow-up time,operation time,preoperative and postoperative follow-up VAS score,ODI score(%),lumbar spine JOA score were recorded.The suture group additionally recorded the height of the intervertebral space in the MRI lateral radiograph before and after the last follow-up.The area ratio of the lateral disc tissue of the T1 weighted phase.The scores of the annular incision group and the inline incision group were compared between groups and groups;the scores of the suture group before and after the operation,the height of the intervertebral space in the MRI lateral slice,and the side of the T1 weighted phase The area of the disc tissue was compared within the group.The tissue area ratio of the lateral disc of the T1 weighted phase=the tissue area of the lateral dise of tho T1 weighted phase/(the area of the upper and lower vertebral body+the tissue area of the lateral dise of the T1 weighted phase)×100%.The height of the intervertebral space,the area of the intervertebral disc and the area of the upper and lower vertebral bodies were obtained by using the quick selection tool and image calculation and measurement tools in Photoshop software.ResultsAll the patients in the three groups were followed up.Because the number of patients in the suture group was too small,the data of the suture group were only compared within the group.The VAS scores of the postoperative follow-up were found by analyzing the preoperative and postoperative scores of the suture group.Lumbar spine JOA score and ODI score(%)were signifieantly-improved compared with preoperative(P<0.05).The height of intervertebral space at the last follow-up of suture group was not significantly decreased before operation.The area of intervertebral disc tissue did not change significantly before surgery(P>0.05)There were no significant differences in follow-up time,gender composition,age,preoperative VAS score,preoperative lumbar spine JOA score,and preoperative ODI score(%)between the annular incision group and the inl.ine incision group(P>0.05).The VAS score,lumbar spine JOA score and lumbar ODI score(%)were significantly improved at different time points after operation(P<0.05).There were no significant differences between the two groups before and after the operation(P>0.05).Complications and recurrence:1 case of post-peritoneal hematoma complications,1 case of recurrence,and 1 case of recurrence after conservative treatment;1 case of ring incision recurrence,no recurrence after reoperation;no suture group during follow-up Recurrence and complications.ConclusionThrough the clinical efficacy observation,the treatment of LDH by circular incision and in-line incision,the treatment of LDH can obtain good short-term therapeutic effect,but the long-term effect is still unclear,so it takes a longer follow-up process to provide More reliable data support.Through the clinical observation of the patients in the suture group,the annulus fibrosus can also have a good therapeutic effect in the treatment of LDH,and at the same time slow the degeneration of the intervertebral disc.Due to the small number of cases in this suture group,subsequent support is still needed for large sample cases.This experiment can accurately measure the height of the vertebral body and the area of the intervertebral disc with irregular shape compared with the previous research method through the quick selection tool and calculation tool in Photoshop software. |
