ObjectiveTo explore the difference of curative effect of Fu' s Subcutaneous Needling at different intervention timing in the treatment of primary dysmenorrhea, and to improve the clinical efficacy.MethodsSixty-six volunteers with primary dysmenorrhea were recruited and randomized into pre-treatment group(treated with Fu' s Subcutaneous Needling 3-7 days before menstruation)and immediate group(treated with Fu ' s Subcutaneous Needling on the day of onset of dysmenorrhea),both groups are treated once every other day and twice a month for a total of three menstrual cycles.Volunteers have scored VAS and CMSS scores at the end of each menstrual period in 1 month before treatment, during treatment in the three menstrual cycles and after treatment in the first menstrual cycle(as follow?up data). After the follow-up study,a statistical analysis of the data was carried out to clarify the difference in the efficacy of different intervention timing of the Fu's Subcutaneous Needling.ResultsAfter 3 menstrual cycles, the VAS score,CMSS duration score and CMSS severity score of the two groups were better than those before treatment, and the difference was statistically significant(P<0.05).Comparing the two groups, the P values of pre-and post-treatment for the VAS score difference and the CMSS duration score difference were 0.041 and 0.048 respectively, both were statistically significant. And the difference of the two scores in the pre-group was higher than that in the immediate group.Besides, there was no significant difference of CMSS severity score difference between groups(P>0.05).There were also differences in onset time, cumulative effect and the follow-up situation between the two groups. In the comparison of onset time, the three indicators of the pre-treatment group were all effective in the first month of treatment(P<0.05,in the comparison with the data before treatment). But in the immediate group, only VAS score was effective in the first menstrual cycle of treatment, while CMSS duration score and CMSS severity score didn' t take effect until the 2nd and 3rd menstrual cycles respectively. In the comparison of the cumulative effect of treatment,it is observed that the mean and median of the three indicators in the pre-treatment group decreased month by month during the treatment,so the cumulative effect of treatment was stable. There was no cumulative effect on the three indicators in the immediate group, and the mean and median of the CMSS score shows a"decrease-increase-decrease" trend in the broken line chart, and so no cumulative effect was observed. The effect is unstable. In the comparison of follow-up study, all three indicators of the pre-treatment group maintained the curative effect after treatment(P<0.05 in comparison with the data before treatment and during the follow-up period);in the immediate group, only CMSS severity score and VAS score maintained the curative effect(P<0.05 in comparison with the data before treatment and during the follow-up period),while CMSS duration score rose back to the level before treatment.ConclusionFu's Subcutaneous Needling therapy is effective in treating primary dysmenorrhea.Pre-treatment with Fu's Subcutaneous Needling has better clinical effect than immediate treatment on primary dysmenorrhea. It can relieve pain better, shorten the duration of dysmenorrhea-related symptoms more significantly,at the same time,it has quicker onset, more stable curative effect and maintains better curative effect after treatment.Based on this study,Fu' s Subcutaneous Needling probably helps to prevente the occurrence of primary dysmenorrhea, but it still needs further study.
