Objective:This study researched Primary Dysmenorrhea(PD),the PD patients as the experimental group,compared with the healthy people.We used puncturing sanyinjiao(SP6)as the treatment therapy,and used functional magnetic resonance imaging(fMRI)as the technology of extracting,build brain response characteristics of puncturing SP6,and used hippocampus as the seed conduct resting state functional connectivity,to investigate the central mechanism of acupuncture in the treatment of PD,and hoped to provide scientific references for the further research and clinical application.Methods:This study included 10 PD patients and 10 healthy people,after one menstrual cycle baseline investigation,all PD subjects would receive an immediately acupuncture treatment when the VAS?40mm in the first to third day during the menstrual period.Clinical evaluation and resting state fMRI scans were performed on PD group before and after treatment.All healthy subjects proceeded f MRI scan with the PD patients at the same time.After all scanning finished,we used the hippocampus as the seed point,calculated the resting state functional connectivity degree,and compared the fMRI image before and after the treatment,to analyze the changes of brain function and functional connectivity in PD patients.Results:1.Puncturing SP6 can mitigate the pain intensity of PD patients,contrast before and after acupuncture,the grades of the VAS were decreased(P < 0.05,statistically significant).2.Compared the PD patients and the healthy people,we found that the functional connectivity of the right hippocampus exhibited hyperconnectivity in the OFC,and the thalamus,the insula,the postcentral gyrus showed lower connectivity.The functional connectivity of left hippocampus exhibited hyperconnectivity in the OFC,and the thalamus,the insula,the postcentral gyrus,the right hippocampus showed lower connectivity.3.The functional connectivity of hippocampus was changed before and after the acupuncture treatment.When we used right hippocampus as the seed point,hyperconnectivity exhibited in the left posterior central gyrus,the right anterior central gyrus,and the right angular gyrus;the left OFC,the left mPFC and the left amygdala showed lower connectivity.When we used left hippocampus as the seed point,we found the left thalamus,anterior central gyrus,posterior central gyrus,the right angular gyrus showed the hyperconnectivity;the left OFC connection degree reduced.Conclusions:1.Puncturing SP6 can relieve the pain intensity of the PD patients.2.The changes of rs-fc between the limbic system and hippocampus may be involved the central mechanism of the cause of primary dysmenorrhea;3.The changes of rs-fc between somatosensory association cortex,the limbic system and hippocampus may be related to the central mechanism of acupuncture treatment of primary dysmenorrhea.
