Objective:To evaluate the clinical efficacy and safety of Shugan Wendan decoction on glucose and lipid metabolism in patients with metabolic syndrome,and to provide scientific basis for the prevention and treatment of metabolic syndrome diseases by traditional Chinese medicine.Methods:100 patients with metabolic syndrome of phlegm and blood stasis combined with stagnation of liver-qi were randomly divided into the experimental group and the control group by using the blind method of envelope uncovering according to the proportion of 1:1.The control group was treated with routine western medicine for 12 weeks without taking any traditional Chinese medicine or proprietary Chinese medicine.On the basis of routine western medicine treatment,the experimental group was given traditional Chinese medicine granules:Shugan Wendan decoction granule 5 packets per week(1 time per day,5 days per week),continuously administered for 12 weeks.Total cholesterol(TC)、triglyceride(TG)、high density lipoprotein cholesterol(HDL-C)、non-high density lipoprotein cholesterol(nonHDL-C)and low density lipoprotein cholesterol(LDL-C)、Glycosylated erythroprotein(HbA1c)、fasting plasma glucose(FPG)、fasting insulin(FIN)、insulin resistance index(HOMA-IR)、hypersensitive c reactive protein(hs-CRP)、tumor necrosis factor-α(TNF-α)、Traditional Chinese Medicine(TCM)syndrome score、Changes of clinical efficacy and safety index of TCM were observed before and after treatment.Results:Before treatment,there was no significant di fference in TC、TG、IIDL-C、nonHDL-C、LDL-C、HbA1c、FPG、FLN、HOMA-IR、hs-CRP、TNF-a、TCM syndrome score between the two groups(P>0.05).After treatment,TC、TG、nonHDL-C、LDL-C decreased and HDL-C increased after treatment.The difference of TC、TG、HDL-C、nonHDL-C、LDL-C between the two groups before and after treatment was not statistically significant(P>0.05).In the aspect of glucose metabolism,the HbAlc、FPG、FIN,HOMA-IR of the two groups decreased after treatment,and the difference of FPG、FIN,HOMA-IR between the two groups before and after treatment was statistically significant(P<0.05);and the difference of HbAlc between the two groups was not statistically significant(P>0.05).In terms of inf lammation,hs-CRP、TNF-a of the two groups decreased after treatment,and the difference of hs-CRP between the two groups before and after treatment was not statistically significant(P>0.05),and the difference of TNF-a between the two groups before and after treatment was statistically significant(P<0.05).In terms of clinical efficacy of TCM,the total integral of TCM syndromes in the two groups decreased after treatment,and the difference between the two groups before and after treatment was statistically significant(P<0.05).The total effective rate of the experimental group was 87.23%and the total effective rate of the control group was 59.57%.The clinical efficacy of TCM in the experimental group was higher than that in the control group(P(0.05).The safety indexes were not abnormal and no adverse events occurred in the two groups before and after treatment.Conclusions:Shugan Wendan decoction can improve the metabolism of glucose and clinical syndrome of TCM in patients with metabolic syndrome.Its mechanism may be related to the improvement of insulin resistance and the alleviation of inflammatory reaction,and the safety is high.
