The Effect Of Tianma Jiannao Decoction On The Symptoms And Cerebral Blood Flow Of Phlegm-dampness Intermediate Resistance PCI Vertigo

Objective:In this study,Tianma Jiannao decoction,which has been used for a long time and has good curative effect,was used to observe the effect of Tianma Jiannao decoction on patients with posterior circulation ischemic vertigo of phlegm-dampness obstruction type,to evaluate the effectiveness of Tianma Jiannao decoction and its impact on quality of life,and to provide basis for follow-up study.Methods:According to the order of visiting sequenobstruction ce,patients with posterior circulation ischemic vertigo of phlegm-dampness type were divided into two groups:treatment group(40 cases)and control group(40 cases),with a total of 80 cases.The general data of gender and age of the two groups were processed by statistics,and there was no significant difference and comparability.The two groups were given basic treatment,including anti-platelet aggregation,lipid-lowering and plaque-stabilizing and basic disease treatment;the control group was given betahistine mesylate tablets,12 mg tid;the treatment group was given Tianma Jiannao decoction,one dose,twice a day;the observation lasted for 14 days.Before and after treatment,liver and kidney function,blood lipids and TCD were examined respectively.General data of patients were recorded.DHI-S,DARS,Chinese Medicine Symptoms Score Table and mRS Score were completed by two physicians.Data were analyzed and processed by SPSS21.0 software.Results:After 14 days of treatment,statistics show that Tianma Jiannao decoction can effectively improve the PCI vertigo and improve the cerebral blood flow supply.1.In the aspect of TCM symptoms,compared with the scores before treatment,the scores of the two groups after treatment were lower,and there was significant difference between the two groups after treatment(P<0.01).The treatment group was better than the control group(P<0.01).According to the evaluation criteria,two patients in the treatment group were cured,24 significantly effective,8 effective and 6 ineffective;one in the control group was cured,7 markedly effective,17 effective and 15 ineffective.The total effective rate was 90%in the treatment group and 62.5%in the control group.The results showed that there were significant differences between the two groups(P<0.01).Tianma Jiannao decoction was better than the control group in improving symptoms.2.In terms of TCD,the average blood flow velocity of vertebrobasilar artery increased and pulsation index decreased in both groups after treatment,suggesting that both methods can effectively improve cerebral blood supply(P<0.01).After treatment,compared with the control group,the average blood flow velocity and pulsation index of vertebrobasilar artery in the treatment group improved better,and Tianma Jiannao decoction could better improve the cerebral blood flow supply(P<0.01).3.DHI-S,DARS and mRS scores improved after treatment in both groups(P<0.05).After treatment,there were significant differences between the two groups(P<0.01).The treatment group was significantly better than the control group.Tianma Jiannao decoction could better improve the symptoms of vertigo and improve the quality of life of patients.4.In terms of blood lipids,there was no significant difference between the two groups before and after treatment(P>0.05).During the treatment period,there was no adverse reaction between the two groups.There was no significant difference in liver and kidney function between the two groups(P>0.05).It was considered that both drugs were safe and feasible.Conclusion:Because of the complexity of symptoms and signs,the diagnosis and treatment of posterior circulation ischemia are difficult.The treatment of posterior circulation ischemia is mainly based on the reference of anterior circulation.However,there is insufficient evidence and treatment guidelines for posterior circulation ischemia.At present,people have increasingly recognized the importance of posterior circulation ischemia,but there are still few targeted studies,which is also the direction of future efforts.By comparing Tianma Jiannao decoction with betastine mesylate Tablets,this study concluded that Tianma Jiannao decoction could better improve vertigo symptoms,blood supply and quality of life of patients.The principle of syndrome differentiation and treatment in traditional Chinese medicine has unique advantages in the treatment of vertigo.
