Correlation Between TCM Syndrome Types And Amino Terminal B-type Natriuretic Peptide Precursor In Acute Cerebral Infarction

Objective:By studying the correlation between TCM syndromes of acute cerebral infarction and amino terminal B-type natriuretic peptide precursor(NT-proBNP),the clinical feasibility of new objective indicators of TCM syndromes in acute cerebral infarction was discussed.Methods:It was included in the emergency department of the Foshan Chinese Medicine Hospital from June 1,2017 to January 20,2019,and the first diagnosis of acute cerebral infarction in the Department of Emergency Medicine,the first district of Department of Neurology,the second district of Department of Neurology and intensive care department.TCM syndrome differentiation was performed according t o the criteria.NT-proBNP,coagulation function,biochemistry and other related examinations were performed in the inpatient department.The correlation between TCM syndromes and NT-proBNP was explored by stat istical methods.Result:A total of 200 cases wore included in the study,including 121 males(60.50%)and 79 females(39.50%).The male to female ratio was 3:2,with an average age of(68.19±13.56)years.The age range of the number of cases in this study was:younger seniors(40.0%)>seniors(36.5%)>middle-aged(16.0%)>young(5.5%)>long-lived elderly(2.0%).There are more young seniors and elderlys in the onset of stroke,the number is roughly the same,45-89 years old is the most important age group,and the number of young people and Longevity old man is small.In terms of gender,the number of males is significiantly higher than that of females.On the distribution of syndrome type,Qi deficiency and blood stasi s syndrome(32.5%)>Yin Xu Feng Dong syndrome(25.0%)>Tan Re Fu Shi syndrome(15.5%)>Feng Yang Shang Rao sydrome(14.0%)>Zhong Zang FUsydrome(13.0%).Among them,the number of qi deficiency and blood stasis syndrome is the highest,and the number of Zhong Zang FU syndrome is the least.Among the previous diseases of acute cerebral infarction,hypertension accounted for 70.0%,diabetes was 27.5%,coronary heart disease,cerebral infarction accounted for 38.5%and 25.5%,and atrial fibrillation was less,that is 15.5%.The levels of NT-proBNP in each syndrome were:Feng Yang Shang Rao sydrome(241.12±406.14)pg/ml,Qi deficiency and blood stasis syndrome(473.34±657.94)pg/ml,Tan Re Fu Shi syndrome(386.43±553.96)pg/ml,Zhong Zang FU sydrome(2677.77±2169.41)pg/ml,Yin Xu Feng Dong syndrome(753.21±1310.25)pg/ml.The highest level of the NT-proBNP was the Zhong Zang FU sydrome.And the difference was statistically significant(P<0.01).There was no difference in the level of NT-proBNP between the patterns(P=1.00).Two-class logistic regression analysis found that whether there was hypertension,diabetes,cerebral infarction,coronary heart disease,atrial fibrillation,gender(P=-0.031<0.05,95%CI:1.112,9.041)and NT-proBNP(P=0.007<0.05,95%CI:0.081,0.672)was the independent risk factors for the syndrome of Feng Yang Shang Rao sydrome.Corrected whether there was hypertension,diabetes,cerebral infarction,coronary heart disease,atrial fibrillation and gender,NT-proBNP(P=0.005<0.01,95%CI:1.944,42.999)was Zhong Zang FU syndrome Independent risk factors.ROC curve analysis found that NT-proBNP was not valuable for the screening of syndromes of Feng Yang Shang Rao sydrome(AUC=0.361,95%CI:0.269,0.453,P=0.018<0.05),which has certain value for screening Zhong Zang FU syndrome(AUC=0.888,95%CI:0.818,0.958,P=0.000<0.001).When NT-proBNP was bounded by 814.50 pg/ml,the accuracy of screening for Zhong Zang FU syndrome was the highest,with an accuracy of 69.7%,a sensitivity of 0.846,and a specificity of 0.851.Conclusion:Among the patients with acute cerebral infarction,young and elderly are predominant,with more men than women.There is a certain correlation between Feng Yang Shang Rao sydrome and Zhong Zang FU sydrome and NT-proBNP in acute cerebral infarction.NT-proBNP is an independent risk factor for Zhong Zang FU sydrome and spleen on acute cerebral infarction.NT-proBNP has certain value for screening Zhong Zang FU sydrome in the sputum,but it is of low value in screening Feng Yang Shang Rao sydrome.
