Objective:The purpose is to analysis pathological changes of N-terminal pro brain natriuretic peptide(N terminal pro brain natriuretic peptide,NT-pro BNP)in acute cerebral infarction(Acute Cerebral Infarction,ACI)patients and correlation with infarction areas,infarction sites,carotid artery atherosclerosis(Carotid artery atherosclerosis,CAA),degree of neural function defect and prognosis are also discussed.Methods:By taking 90 cases of patients with acute cerebral infarction which came from February 2015 to September 2016 in the Second Affiliated Hospital of Bengbu Medical College Department of Neurology as case group,at the same time 40 cases of healthy volunteers as matched group.We check up and compare the level of plasma NT-pro BNP between case group and matched group on admission of 24 hours,treatment of 7 days and treatment of 14 days.According to the result of head MRI,we also compare the level of plasma NT-pro BNP in case group in accordance with the area of infarction and the site of infarction on admission of 24 hours,the treatment of 7 days and treatment of 14 days;we use color Doppler ultrasound to detect carotid artery intima media thickness(intima media thickness,IMT)in case group and compare IMT with the level of plasma NT-pro BNP on admission of 24 hours;by referring to the National Institutes of Health Stroke Scale(the National Institutes of Health Stroke Scale,NIHSS),we divide patients in case group into groups in accordance with neural function defect degree and compare them with the level of plasma NT-pro BNP on admission of 24 hours for the sake of tracking prognoses.Results: The level of plasma NT-pro BNP in case group were significantly higher than matched group(t=16.840,P=0.00),it reached the highest level on admission of 24 hours and then gradually decreased,it dropped to the minimum in 2 weeks;the level of plasma NT-pro BNP on admission of 24 h were significantly higher in the patients with severe sub group than those in the mild group,and the level of plasma NT-pro BNP in the mild group were higher than those in the healthy matched group(F=102.386,P=0.00)and the prognosis was gradually improved;the level of plasma NT-pro BNP in large area cerebral infarction group was significantly higher than that in medium areainfarction group on admission of 24 hours,the treatment of 7 days and 14 days,and the level of plasma NT-pro BNP with medium cerebral infarction area was significantly higher than that in small area infarction group(F=188.694?200.641?181.731,P all=0.00);the level of plasma NT-pro BNP in Anterior circulation cerebral infarction and Posterior circulation cerebral infarction had no statistically significant difference(F=0.000,P>0.05);the level of plasma NT-pro BNP on admission of 24 hours in cerebral infarction group is positively correlated with NIHSS and IMT(r=0.692?0.672,P all=0.00).Conclusion: In acute stage,The level of plasma NT-pro BNP in patients with acute cerebral infarction were significantly increased.The greater of infarct size,the higher increased the value of NT-pro BNP,but it had nothing to do with the infarction site,plasma NT-pro BNP levels is of great significance in the prediction on carotid atherosclerosis risk,It also has important reference value in predicting the course and outcome of this disease.As an important indicator for monitoring acute cerebral infarction dynamically,we combine the level of plasma NT-pro BNP on admission of 24 hours with NIHSS scores which could be more fully and powerfully to estimate severities and prognoses of acute cerebral infarction.
