With the increase in the social general improvement of living standards and the acceleration of aging,Chronic diseases have always been the focus of attention.Diabetes is one of the most important chronic non-communicable diseases that are dangerous to human health at present,and the annual medical expenditure for diabetes and related diseases in China is also very high,so it is urgent to effectively curb the development of diabetes.Among the patients with diabetes,the prevalence of type 2 diabetes is as high as 90%.As type 2 diabetes patients are greatly affected by psychological factors,the importance of lifestyle intervention for the treatment of type 2 diabetes has far exceeded the medical treatment itself.This paper explored the self-management status of type 2 diabetes patients from the perspective of social capital,and conducted a questionnaire design on the basis of reviewing relevant literature and hypothesizing.The self-management model of type 2 diabetes mellitus was constructed.We use exploratory factor analysis to identify six common factors of social capital:social support,social network,social belonging,social participation,social trust and emotional stress regulation,and through the descriptive statistics analysis,diffierence of examination to know some basic laws of social capital and self-management.Finally,the structural equation model is evaluated.The research showed that social participation has no significant influence on patients' self-management,and social support,social affiliation and emotional stress regulation all have significant positive influence on patients' self-management,and social network has a significant negative influence on patients' self-management.In this paper,a model of type 2 diabetes self-management is established in theory and relevant suggestions can be put forward for the self-management mechanism of diabetic patients in practice.
