ObjectiveTo explore the effect of family-oriented chronic disease management on the self-management,quality of life and emotion in COPD patients by setting up family-oriented chronic disease management to intervene the family supportive system in COPD patients.MethodsOne hundred patients with COPD recruited from The Affiliated Hospital of Qingdao University were assigned chronologically according to the date of hospital admission.Patients recruited from November 2013 to January 2015 were assigned to the control group given patient-centered chronic disease management,while those recruited from February 2016 to February 2016 were assigned to experimental groups with family-oriented chronic disease management incorporating partnership with patients,their spouse and children into daily care.There were 50 patients in each group.The time course for the study of both group lasted for 6 months.The self-management scale,St.George's Respiratory Questionnaire(SGRQ)and 6 minutes walking test(6MWT)were used for the evaluation of the self-management ability and quality of life of those patients.Family support sclaes were applied to evaluate the effect of family support in the experimental group.All data are analyzed by SPSS 17.0 software kit.The nominal-scale data are analyzed by ?2 test,while the quantitative data are done by t test.ResultsFifty subjects were enrolled as controls with average age of 65.46±6.49 years,among which were 36 male and 14 female.Another 50 patients were enrolled in the experimental group with the average of 66.00±4.61 years,among which are 38 male and 12 female.Statistical results showed that:1.There were no differences in the general information regarding patients' gender?age?educational background?family monthly income?severity of diseases? scale of dyspnea?self-care ability?exerciseability and self-management ability between the two groups befor the intervention(P<0.01).2.Result of self-management ability assessment:after 6 months intervention parameters of self-management ability in the experimental group including symptom management,daily life score,emotional management score,information management score,self-efficiency score and self-management score were 29.54±2.40,49.16±5.59,45.52±3.91,20.28±2.72,32.38±3.21,176.88±11.36 respectively,while those in the controls were 23.02±2.72,41.92±5.01,37.34±4.87,15.88±3.09,24.84±3.84 and 143.0±12.99.The parameters in the experimental group were significantly high than those in the conteols(P<0.01).3.The results of quality of life assessment: the post-interventional parameters of quality of life including total SGRQ score,scores of symptom,activity score,impact score and 6MWT distance in the experimental group were 37.59±9.03,37.5±8.64,37.6±11.18,37.5±10.81 and 349.18±19.62 respectively,while those in the controls were 47.58±6.72,46.90±6.93,49.65±8.78,46.83±7.78 and 285.24±19.79 respectively.Those parameters in the experimental grops were significantly better than those in the control groups(P<0.01).4.Results of the emotional assessment: the post-interventional emotion scores including anxiety score and depression score assessed by the hospital anxiety and depression scale(HAD)in the experimental group were 7.42±2.88 and 6.04±1.82 respectively,which were significantly decreased than those in the controls being 9.34±3.37 and 7.52±2.02 respectively(P<0.01).5.The results of the family support in the experimental group showed a significant increase in the family support scale after the intervention(13.22±1.28)in comparison to that before the intervention(10.92±2.86)(P<0.01).ConclusionFamily-centered chronic disease management intervention for family supportive system in COPD patients can improve self-management skill,quality of life and emotion.The establishment of the health support system implies significant promotion for the effective chronic management of COPD by the co-setup of the medical professionals and the patients,and their families as well,in the context of building a perfect community care.
