Objective: Five neuropsychological scales were used to evaluate the characteristics of emotional regulation and cognitive strategies in patients with Wilson's disease(WD),and to analyze the influencing factors and occurrence mechanism,and to explore the characteristics of emotional regulation and cognitive strategies in TCM syndromes of WD patients.Methods:Seventy-eight patients with WD were divided into liver type group(16 cases)and brain type group(62 cases)according to their clinical manifestations,and 84 cases in healthy control group.The results of gender,age,educational years and Barratt Impulse Scale(BIS-11-C),Buss-Perry Aggression Scale(AQ-C),Emotional Intelligence Scale(EIS),Cognitive Emotional Regulation Questionnaire(CERQ-C)and Toronto Emotional Disorder Scale(TAS-20)were recorded in detail.Brain magnetic resonance imaging(MRI)results and clinical data of WD patients were recorded.At the same time,WD patients were divided into two types according to TCM syndrome differentiation: liver-qi inversion syndrome(35 cases)and Liver-qi Depression Syndrome(43 cases).The scales of WD patients and healthy control group,WD patients with liver type group,brain type group and healthy control group,liver-qi inversion syndrome group and Liver-qi Depression Syndrome group were compared and related factors were analyzed.Results:(1)The scores of BIS-11-C,AQ-C,EIS,CERQ-C and TAS-20 in WD group were significantly different from those in healthy control group(p<0.05).There were significant differences in BIS-11-C,AQ-C and EIS scores between brain-type and liver-type patients(p<0.05).Frontal lobe(B=10.263,P=0.008)and globus pallidus(B = 8.583,P=0.002)injury were the influencing factors of BIS-11-C score.Thalamus(B=23.477,P=0.000)and caudate nucleus(B=10.030,P=0.017)injury were the influencing factors of AQ-C scores.The course of disease(B=-0.100,p=0.030),modified Young scale score(B=-0.493,p=0.013),frontal lobe(B=-3.328,p=0.001),globus pallidus(B=-3.041,p=0.029)injury were the influencing factors of EIS score.Cognitive emotion regulation strategies of WD patients were affected by gender(B=1.524,P=0.030),years of education(B=0.497,P=0.002),course of disease(B=0.105,P=0.035),putamen(B=-2.348,P=0.011),thalamus(B=2.058,P=0.037),frontal lobe(B=-2.142,P=0.035).Age(OR=0.819,P=0.009),course of disease(OR=1.223,P=0.003),frontal lobe(OR=9.377,P=0.010),putamen(OR=5.164,P=0.026)injury are the influencing factors of alexithymia in WD patients.(2)The scores of BIS-11-C,AQ-C,EIS,CERQ-C and TAS-20 in WD patients with different TCM syndromes of Liver-qi inversion and Liver-qi Depression were significantly different(p<0.05).Conclusion: WD patients have lower level of emotional intelligence,higher impulsiveness and aggression,higher incidence of alexithymia,and more self-accusation,meditation and catastrophic strategies when encountering negative life events.The emotional intelligence level of brain type patients was lower than that of liver type patients,and impulsiveness and aggressiveness were higher.Emotional regulation and cognitive strategies of patients are related to TCM syndrome types.Gender,age,course of disease,years of education and brain structure damage have different effects on emotional regulation and cognitive strategy choice.
