Objective : To observe the clinical efficacy of sinew-regulating and bone-setting manipulation(SRBSM)combined with exercise therapy(ET)in the treatment of female patients with chronic neck pain, and to explore it's mechanism. Methods:Sixty female patients with chronic neck pain who met the inclusion criteria were randomly divided into the treatment group(30 cases)and the control group(30cases).Patients in the treatment group were treated with SRBSM combined with ET,while those in the control group were treated with modulated medium frequency current therapy(MMFCT)and ET.SRBSM was performed once every other day, three times a week, and the treatment lasted for 5 weeks.MMFCT was performed once a day,6 times a week for 5 weeks.Exercise therapy once a day,6 times a week for five weeks.Visual Analogue Scale VAS,the pressure meter, surface median frequency MF,root mean square RMS,cervical muscle endurance, and Northwick Park Questionnaire(NPQ)were used to evaluate the patient's neck function before and after treatment. The efficacy was observed with diagnosis and efficacy standards of TCM and the treatment for patients after 1 month of follow-up VAS score and NPQ scale score. The assessment process and statistical process are blinded.Results:Finally,57 patients completed the treatment,29 in the treatment group and 28 in the control group.Compared with the same group before treatment,VAS score decreased, tenderness value increased, abnormal surface tension of neck muscle decreased,RMS increased,MF increased, endurance duration prolonged, and the Northwick Park Questionnaire score dropped(all P<0.05).The treatment group was superior to the control group(P < 0.05).The total effective rate was 93.1% in the treatment group, higher than that in the control group(82.1%)(P < 0.05).Follow-up of57 patients showed that VAS and NPQ scores in the treatment group were still lower than those before and after treatment, while those in the control group were only lower than those before treatment, and the results in the treatment group were better than those in the control group, the difference was statistically significant(P < 0.05).Conclusion : SRBSM combined with ET could effectively relieve pain and improve cervical vertebra dysfunction in female patients with chronic neck pain,Its efficacy is superior to MMFCT combined with ET,and is conducive to the maintenance of the efficacy. This is associated with increased pain threshold, increased neck muscle endurance and recruitment, and decreased fatigue.
