Study On The Correlation Between Self-efficacy,Social Support And Burden Of Care In Coma Patients With Traumatic Craniocerebral Injury

Objective:To investigate the currentstatus of self-efficacy,social support and burden level of caregivers in patients with traumatic brain injury(TBI)coma,and to explore the correlation between self-efficacy,social support and burden level of caregivers,so as to improve the care effect and level of caregivers in patients with TBI coma,and formulate relevant measures to provide reference for improving the outcome and prognosis of patients.Methods:The main caregivers of TBI coma patients diagnosed by neurosurgery in three tertiary and first-class general hospitals in Nanchang City of Jiangxi Province were selected as the research objects.The data were collated by SPSS 21.0 and AMOS 21.0 software,using survey tools including general data questionnaire,Zarit caregiver burden scale(ZBI),general self-efficacy scale(GSES),social support rating scale(SSRS).Analysis.In this study,frequency,percentage,standard deviation,t test,variance analysis,Pearson correlation analysis and multiple regression analysis were used for statistical analysis.Results:(1)The total score of caregivers' care burden was(28.53±8.26),which was a moderate to low level.Among them,the score of personal burden dimension was(7.80±2.18),the score of role burden dimension was(18.33±5.22)and the total score of nursing burden was(2.40±0.85).Univariate analysis showed that there were significant differences in the time of coma,the role of patients as the main source of family income,and the age of caregivers(P < 0.05).The results of multiple linear regression analysis showed that the protective factor of care burden was that the patient was married,the educational level of the patient was below junior high school,the age of the caregiver was less than 60 years old,and the care time of the caregiver was less than 8 hours a day.Patients are the main source of income for the family and an independent risk factor for care burden.(2)The self-efficacy score of caregivers was(22.61 +6.16),which was in the middle level of self-efficacy.Univariate analysis showed that there were significant differences in caregiver self-efficacy with,length of patient stay,patient relationship and gender(P < 0.05).The results of multiple linear regression analysis showed that the per capita disposable monthly income of caregivers was more than 1000 yuan.The living city of caregivers was the protective factor of caregivers' self-efficacy,and the male caregivers was the risk factor of caregivers' self-efficacy.(3)The total score of social support of caregivers was(27.57 +4.21),which was in the middle level.Univariate analysis showed that the social support of caregivers had significant differences in payment mode,age and gender of medical expenses(P < 0.05).The results of multiple linear regression analysis showed that the payment mode of medical expenses,hospitalization time,patient occupation and caregiver's educational level,the per capita disposable monthly income of the family,and the place of residence were not independent influencing factors of the caregiver's social support.(4)Care burden was negatively correlated with social support and self-efficacy.Multivariate linear regression analysis showed that Social support is the protective factor that affects the burden of care.Consequence:(1)The main caregivers of TBI coma patients have a mild burden of care and a moderate sense of self-efficacy,but the social support of caregivers is at a moderate to low level.(2)Social support was negatively correlated with the burden of care,while self-efficacy was negatively correlated with the burden of care.(3)The time of coma and the age of caregivers are the risk factors affecting the burden of care.The social support of caregivers and the main source of non-family income of patients are the protective factors affecting the burden of care.(4)Practical help should be given to the caregivers' own health,individual psychological counseling should be used to improve their psychological status,targeted health education should be carried out for caregivers of different ages and family economic conditions,and the contents of coping styles and coping skills should be trained.(5)From a social point of view,we should actively develop community care system,strengthen government support,improve social welfare policies,actively promote the development of welfare institutions and charities,and develop multi-channel absorption of funds for the medical,nursing and rehabilitation of TBI.
