| Research Purpose:To describe the burden of care,social support and quality of life of primary caregiver of patients with primary liver cancer.To explore the influencing factors of primary caregiver’s quality of life.To analyze the correlation between primary caregivers’ social support,burden of care and quality of life;To investigate the mediating effect of social support between burden of care and quality of life.Research Methods:From November 2018 to September 2019,306 primary liver cancer patients and their primary caregivers were conveniently selected from 3 Grade A tertiary hospitals in Shanghai.The research tools include general information,the Caregiver Burden Inventory(CBI),the Social Support Scale(SSRS),and the Caregiver Quality Of Life Scale(CQOL).The date were conducted by SPSS21.0 software.The main statistical methods used including:descriptive analysis,t-test,ANOVA,correlation analysis,regression analysis and mediation effect test.Research Results:1.The average score of care burden of primary caregivers was(33.96±11.19),which was at the level of moderate burden.The average score of social support was(33.39±6.63),which was below the average level.Compared with the national norm,all dimensions and total scores were lower than the national norm.The average score of quality of life was(149.67±31.45),and the four dimensions from high to low were mental health(52.81±9.57),physical health(40.09±10.00),social health(34.68±12.14)and psychological health(22.07±12.16).2.The single factor analysis results show that the main caregivers quality of life scores in patients with different liver function classification,number of hospitalization,treatment methods and the main caregivers different age,sex,relationship with patients,degree of culture,work status,family income,health status,whether suffering from chronic diseases,with or without other caregivers,daily care time,economic pressure differences were statistically significant(P <0.05).3.The results of multiple linear regression showed that the caregiver’s health level,social support level,care burden level,daily care time,economic pressure or not,and the number ofhospital admissions were independent influencing factors for the quality of life of the main caregivers,which explained 38.2% of the variation.4.Social support had a negative correlation with the care burden of primary caregivers(r=-0.429,P<0.05),whereas a positive correlation with the quality of life(r= 0.351,P<0.05).The care burden had a negative correlation with quality of life(r=-0.496,P<0.05).5.Effect of care burden on quality of life of primary caregivers of primary liver cancer patients was mediated by social support and the 95% confidence interval did not include 0(-1.305,-0.724),the mediating effect of social support was 27.04% of the total effect.Research Conclusions:1.The level of care burden of primary caregivers of primary liver cancer patients were heavy,the level of quality of life were low,especially the mental health is not optimistic;The level of social support of primary caregivers was middle,which indicated that hospitals,families and communities should pay more attention to the quality of life of primary caregivers of patients with primary liver cancer,improve the level of social support and reduce the burden of caregivers.2.Primary caregivers’ own health status,social support level,care burden level,daily care time,financial pressure,and the number of hospital admissions were the major factors affecting quality of life.3.The primary caregivers of primary liver cancer patients who had the higher degree of social support,the lower the burden of care and the better the quality of life;The higher degree of the care burden,the lower level of quality of life.4.Social support is a mediating factor that regulates the care burden on the quality of life of primary caregivers of patients with primary liver cancer,which explained that When medical workers reduce the burden of primary caregivers of patients with primary liver cancer,they can take social support as a starting point,make full use of hospital 、family and social resources,improve the level of social support of caregivers,reduce the burden of caregivers,and improve the quality of life of caregivers. |
