Research objectivesThe purpose of this study was to investigate the value of NLR,PLR and RDW in inflammatory bowel disease.Research methods190 patients with inflammatory bowel disease were recruited in the research from January 2016 to December 2018 in our Hospital,including 31 Crohn's disease and 159 ulcerative colitis.There were 31 cases of crohn's disease,10 cases in remission,6 cases in mild active period,11 cases in moderate active period,and 4cases in severe active period according to the CDAI score;There were 159 cases of ulcerative colitis,29 cases of remission,46 cases of mild active period,49 cases of moderate active period and 35 cases of severe active period according to the modified Mayo score.A total of 100 cases of age-matched and gender-matched control group were enrolled in the physical examination center of our hospital during the same period.The differences of NLR,PLR and RDW in inflammatory bowel disease and control group were observed,and the changes of NLR,PLR and RDW in different stages and different severity of Crohn's disease and ulcerative colitis subgroup were analyzed.In addition,the correlation between the above indicators and disease activity scores,erythrocyte sedimentation rate,and C-reactive protein was analyzed by Spearman,and the ROC curve was used to predict the application value of NLR,PLR and RDW in IBD disease activities.Research results:(1)The levels of NLR,PLR,RDW in the IBD group were significantly higher than those in the control group(2.91 vs 1.66,178.22 vs 67.21,13.00 vs 12.30,P <0.01).(2)The levels of PLR and NLR were obviously increased in IBD active period as compared with the remission and control group(189.85vs119.75vs67.21,P <0.01);(3.29vs2.26vs12.30,P <0.01),RDW in active period were increased as compared with control group(13.00 vs 12.30,P <0.01).(3)To compared with the control group,the levels of PLR,NLR and RDW were increased significantly in UC group.PLR and NLR in active period were remarkably higher than those in remission and control group(190.00vs133.13vs67.21,P <0.01);(3.18vs2.29vs1.66,P <0.01),RDW in active period was increased as compared with the remission and control group(13.95vs12.30,P <0.01)(13.95vs12.95,P <0.05).(4)The levels of NLR,PLR and RDW in the CD group were significantly higher than those in the control group(P<0.01).NLR and PLR in the active period of CD were higher than those in the control group(3.70 vs 2.20vs1.66,P < 0.05)(184.64vs115.25vs67.21,P <0.05).RDW was higher in the active period and remission than in the control group(P <0.05).(5)In UC group,the levels of NLR and PLR in the moderate active period and severe as compared with the remission were significantly higher(3.57,3.37vs2.29;P<0.01)(219.73,259.49 vs 133.13;P <0.01),NLR and PLR were significantly increased in severe active period were significantly higher than the mild(3.37 vs 2.29,P <0.01)(259.49 vs 219.73,P <0.01),and there was no significant difference in RDW(P > 0.05).(6)No significant differences in the different severity of NLR,NLR and RDW in the CD group(P > 0.05);(7)PLR and NLR were positively correlated with ESR,CRP,and Mayo scores in UC patients(r = 0.476,r = 0.434,r = 0.314;P <0.01)(r = 0.337,r = 0.331,r = 0.321;P <0.01);(8)NLR was positively correlated with erythrocyte sedimentation rate and CRP in patients with CD(r =0.450,r =0.507;P <0.05),and PLR was positively correlated with CDAI score(r =0.358,P <0.05);(9)Using the Mayo score,the Mayo score is greater than 2,and the PLR,NLR,and RDW ROC curves were plotted,When the cut off value was 2.82,the AUC of the NLR was 0.698,the sensitivity was 60.5%,and the specificity was 81.5%(P <0.05);when the cut off value was 212.46,the AUC of PLR was 0.684,the sensitivity was46.5%,and the specificity was 92.6%(P <0.05).Research conclusions:1.NLR,PLR,and RDW were significantly elevated in patients with IBD and can be used for monitoring disease activity.2.The levels of PLR,NLR and RDW were higher in the active than in the remission and control group.With the aggravation of disease severity,PLR and NLR increased.PLR and NLR were positively correlated with erythrocyte sedimentation rate,CRP and Mayo score.3.The NLR in CD was positively correlated with ESR and CRP,and PLR was positively correlated with CDAI score.
