A Study On The Relationship Between Neutrophil-to-Lymphocyte Ratio,Platelet-to-Lymphocyte Ratio And Red Cell Distribution Width And IgA Nephropathy

Objectives:This study aimed to evaluate the relationship between NLR,PLR,RDW and IgA nephropathy.Methods:A retrospective case–control study with IgA nephropathy and healthy controls was performed.We compared the general clinical data between patients and healthy controls,and the correlations between NLR,PLR,RDW and the clinical and pathological features of IgA nephropathy.The ROC curves of NLR,PLR and RDW were drawn to evaluate the best cut-off value.Results:Compared with the healthy controls,the patients with IgA nephropathy had higher levels of NLR,PLR and RDW.The patients were divided into high and low groups according to the median of NLR.The patients with high NLR group,high PLR group and high RDW group had higher creatinine and urea nitrogen,lower eGFR,and higher 24-hour urinary protein in high NLR group and RDW group.The age factors were adjusted by covariance analysis,except that there was no significant difference in the quantity of24-hour urinary protein among NLR groups,urea nitrogen,Scr and eGFR were still closely related to NLR and PLR.There were more cases of renal tubular atrophy/interstitial fibrosis T2 and T3 in high NLR group(?~2=8.152,P<0.05)and high PLR group(?~2=6.126,P<0.05).The patients with renal tubular atrophy/interstitial fibrosis T3 in high RDW group were higher than those in low RDW group(?~2=13.388,P<0.05).Spearman correlation analysis showed that NLR was positively correlated with Scr,BUN,24-hour urinary protein,renal tubular atrophy/interstitial fiber,PLR was positively correlated with creatinine and BUN,RDW was positively correlated with creatinine,all of them were negatively correlated with eGFR.Multiple ordered logistics regression analysis showed that renal tubular atrophy/interstitial fiber was related to the level of NLR and RDW after adjusting for age factors.Conclusions:NLR,PLR and RDW can reflect the severity of IgAN.The higher the NLR,PLR and RDW,the worse the renal function and the renal tubular atrophy/interstitial fibrosis will be,which is helpful for the evaluation of the prognosis of IgAN.Among them,NLR and RDW may be a potential clinical indexes to predict IgAN conditions.
