Relation Between Neutrophil/Lymphocyte Ratio,Platelet Distribution Width And The Severity Of Coronary Artery Disease

Abstract/Summary:
Objective: To determine whether the serum of neutrophil to lymphocyte ratio(NLR)and platelet distribution width(PDW)differ between patients suffering coronary artery disease and normal people,we can investigate the value of NLR and PDW in the diagnosis of coronary heart disease.To analysis the correlation between the serum NLR,PDW and the severity of coronary artery disease in patients with CAD,we can discuss the value of NLR and MPV in predicting the severity of coronary artery disease.Methods: This study uses the method of retrospective research.According to the selection criteria,a total of 532 patients who underwent coronary angiography were enrolled to the study,from November 2015 to February 2017 at the first Hospital of Shanxi Medical University.There were 384 cases of coronary heart disease patients,age from 41 to 85 years old(60.8 ±9.66 years old)and 147 cases of control group,age from 44 to 79 years old(59.2 ±10.35 years old).All the enrolled patients collected venous blood on an empty stomach,examine blood cell analysis,blood biochemistry,etc.The risk factors of coronary artery disease,such as sex,age,smoking,hypertension,diabetes,low density lipoprotein density(LDL-C),high density lipoprotein cholesterol(HDL-C),triglyceride(TG)were further excluded.All patients underwent coronary angiography in hospital.The syntax score was calculated to represent the severity of coronary artery disease,according to the location,type and degree of stenosis of the coronary artery during the operation.Statistical analyses were performed using SPSS19.0 software.All P values were two-tailed,and values <0.05 were considered to indicate statistical significance.Results: 1.There were 532 patients in coronary heart disease group(mean age 60.8±9.66 years,male 261 cases)and 147 cases in control group(mean age 59.2±10.35 years,male 85 cases).The ratio of male,serum PDW and serum NLR in coronary heart disease group was significantly higher than that in control group(p < 0.05).There was no significant difference in age,hypertension,diabetes,smoking,serum MPV,leukocyte count,serum TG,serum HDL-C,serum LDL-C between coronary heart disease group and control group(p > 0.05).2.There was no significant difference in NLR levels between syntax low group and control group(p > 0.05).The PDW and NLR levels in others SYNTAX groups were significant higher than control group.With the increase of SYNTAX score,the level of PDW and NLR increases gradually.Expect that there was no statistical difference in serum NLR between low SYNTAX group and control group and between middle SYNTAX group and high SYNTAX group(P>0.05),other arbitrary groups were s statistical difference(P < 0.05).3.Multiple linear stepwise regression analysis showed that there was a positive correlation between syntax score and serum NLR(t=3.395,P<0.05),serum PDW(t=2.941,P<0.05),serum TG(t=1.675,P<0.05).4.Univariate logistic regression analysis showed PDW(OR=2.104,P<0.05)and NLR(OR=1.125,P<0.05)were predictive factors that SYNTAX score was more than 32.Further multivariate logistic regression analysis showed that serum PDW(OR=2.35,P<0.05)and NLR(OR=1.186,P<0.05)were independent predictors of syntax score > 32.Conclusion:Based on our study,serum NLR and PDW were positively correlated with SYNTAX scores,and were independent predictors of SYNTAX scores more than 32.Early monitoring of serum PDW and NLR levels has a good predictive value for the severity of cad patients,which can provide reasonable suggestions for treatment strategies.
