Objective: Explore percutaneous kyphoplasty(PKP)The feasibility of treating mild,moderate,and severe osteoporotic compression fractures(OVCF)was evaluated.The short-term efficacy was evaluated and the difference in efficacy of OVCF with PKP was analyzed.Methods: According to the exclusion criteria,44 patients with OVCF who underwent PKP treatment from November 2017 to November 2018 in our hospital were divided into mild group(16 cases)and moderate group according to the degree of vertebral compression.Cases,severe group(13 cases),different patients were treated with individualized surgery,different bone cement injection volume,try to avoid serious complications such as bone cement leakage.Oswestry disability index(ODI)questionnaire and visual analogue scale(VAS)method were used on the first day of admission and on the first day,the third day and the first week after surgery.The improvement of daily activity function and the degree of pain relief were evaluated.X-ray examination was performed before and after operation.The height recovery value of the vertebral body leading edge,Cobb angle correction degree and bone cement leakage were analyzed and analyzed.Results: All the cases in this study were successfully performed by the same doctor group.There were no serious complications such as vascular,nerve injury and pulmonary embolism.The average height of the anterior vertebral body before operation was 14.74±4.24 mm,and the average postoperative time was 19.59±3.72 mm.There was a significant difference before and after operation(P<0.001);the preoperative Cobb angle was 19.43±4.68°,and the average postoperative time was14.23±4.10°.There was a significant difference before and after surgery(P<0.001);The height recovery values ? ? of the vertebral body of the degree,moderate and severe group were 4.13±0.72 mm,5.21±0.72 mm and 5.31±1.01 mm,respectively.There was no significant difference between the moderate and severe groups.The mild group and the moderate and severe groups had Significant differences;Cobb angle corrections were 4.24±0.49°,5.61±0.46°,and 5.84±0.50°,respectively.There were no significant differences between the moderate and severe groups.There were significant differences between the mild group and the moderate and severe groups.The largest group was the severe group;the cement leakage rates of the mild,moderate,and severe groups were 12.5%,20.0%,and 38.5%,respectively.The cement leakage rate of the severe group was the highest,the moderate group was the second,and the mild group was the lowest.Three groups of patients with pain visual analogue scale(VAS score)and ODI dysfunction index were admitted to the hospital on the first day of 8.47 26.89 decreased to 1.75,7.14 on the first day after surgery to1.37 and 5.77 at 1 week after surgery.The differences between the three groups were statistically significant.Conclusion: PKP treatment of different degrees of osteoporotic thoracolumbar vertebral compression fractures,postoperative pain was significantly reduced,daily function was partially or completely restored,compared with severe group,mild and moderate group postoperative pain relief,daily life function recovery fast;Compared with the severe group,the mild and moderate group Postoperative pain relief,daily life function recovery is faster;PKP surgery can better restore the height of the injured vertebrae and correct the Cobb angle of the responsible vertebra,in the range of indications,the greater the degree of compression of the vertebral body correction effect have more advantages.In the cement leakage rate,the cement leakage rate of the severe group was the highest,and the mild group was the lowest.In general,the OVCF treatment with different degrees of PKP treatment can achieve good curative effect.It has been widely used in clinical practice,but for different individuals,the surgical plan Should be individualized in order to achieve the best therapeutic effect and minimize the occurrence of postoperative complications.
