China is a developing country with a large population, and people's demands for health care are getting higher and higher with the improvement of economy and living standards.Implementing the signing services for family doctors is the key to strengthening the primary health care system and is the key point to promote the rational allocation of medical resources.Studying the signing services for family doctors can provide theoretical support for the government to implement medical system reform, and also enable people to enjoy better medical service quality with lower medical expenses, which is also the main purpose of this study. In this study, five community health service centers in Huli District were selected as research sites. The method of convenience sampling(accidental sampling)was adopted. The young and middle-aged signing people and unsigning people were the research objects. The data was collected by electronic questionnaire of influencing factors, which was used for statistical analysis. In our results,we found that the current signing rate of young and middle-aged people in Huli District is about 60%.Nearly 70%of the unsigning people expressed their willingness to sign the service after receiving the survey. Most of the signing people have a general degree of implementation and satisfaction and a high willingness to renew their signing.Household registration will affect the willingness to sign the service, monthly income level will affect the degree of implementation, and gender will affect the degree of satisfaction. The medical treatment habits of the first diagnosis to the community health service center, the level of understanding of the family doctor policy, the demand/utilization of the standardized management of chronic diseases, and the demand/utilization of the service of disease rehabilitation will affect the willingness to sign the service, and the degree of implementation and satisfaction. The signing services for family doctors is very popular, but there are still some problems. We recommend encouraging non-local household registration people to complete signing services and increasing treatment and rehabilitation services that are more suitable for young and middle-aged people. In addition, propaganda should be strengthened to make people accustomed to the first diagnosis in community health service centers.
