| Objective:This topic through the study of rhubarb mirabilite dispersion were applied in the CV 8 point and apply the umbilical mixed hemorrhoid postoperative constipation prevention and control of hot and humid betting type,detailed record three groups after treatment in patients with efficient,defecation time,for the first time that constipation related symptoms and postoperative complications,and watch the preoperative and postoperative 7 d the change of plasma SP,VIP,expression,assessment of rhubarb mirabilite scattered apply umbilical prevention and control of the clinical curative effect of treating type mixed hemorrhoid postoperative constipation.Based on the external treatment of traditional Chinese medicine,combined with the characteristics of dispersant and meridians acupoints,the mechanism of rhubarb glauber's powder in preventing and treating postoperative constipation of damp-heat wadding type mixed hemorrhoids was preliminarily discussed,which provided a new idea for reducing postoperative complications and reducing patients' painful state,and provided a basis for the clinical application of rhubarb glauber's powder.Methods: This project observed 60 inpatients diagnosed as wet-heat wadding mixed hemorrhoids and treated with surgery(external stripping and internal ligation)in the department of proctology of the first affiliated hospital of anhui university of traditional Chinese medicine from November 2016 to November 2018.The ratio of male to female was 1:1,and the age distribution interval was 19-57 years old.According to the sequence of admission,60 patients were divided into three groups according to the random number table method,including 20 patients in the treatment group,20 patients in the control group and 20 patients in the blank group.In the treatment group,4-5g prepared rhubarb mirabilite powder was applied to shenque point.In the control group,lactulose was taken orally,10 ml,3 times a day.The blank group had no special drugs in the routine treatment,which guided the patients to defecate regularly and eat reasonably.The gender,age and other general information of the patients in the three groups were recorded in detail.Observe and record the first bowel movement time;Postoperative pain,edema and fluid leakage were recorded on day 3 and day 7.On the 7th day after surgery,constipation-related symptoms and efficacy were inquired and recorded.Plasma SP and VIP levels before and at 7d after surgery;Adverse reactions occurred during the observation of therapeutic effect were completely recorded.According to the statistical software(SPSS 21.0),the data obtained were statistically analyzed and the results were evaluated objectively.Results:Before treatment,the general conditions of the three groups,namely gender and age,were compared in an equilibrium manner,and the difference was not statistically significant(P >0.05),which was comparable.No adverse reactions occurred during the treatment.The research results are as follows: The three groups of patients with efficient comparison: three groups of patients in postoperative constipation prevention and efficient for evaluation on the 7 d,results: the treatment group(90%)> the control group(85%)>(45%)with the blank group,the treatment group and control group there was no difference in the efficiency,there were statistical differences between two groups with the blank group,P < 0.05,indicating that the treatment group and control group in mixed hemorrhoid postoperative constipation prevention effect is better.Evaluation of first bowel movement time: postoperative first bowel movement time of the three groups was(1.988 0.879)d,(2.449 1.644)d,and(3.421 1.166)d,respectively.After statistical analysis,there was a difference among the three groups,P <0.05,indicating that the treatment group was better than the control group in improving the first defecation time,and the control group was better than the blank group.(3)Constipation symptom score evaluation: after statistical comparison of the overall constipation symptoms in the postoperative treatment process,both the treatment group and the control group had better constipation symptom score than the blank group,P <0.05,and the difference was statistically significant.In addition,the treatment group was superior to the control group in defecation difficulty,overexertion defecation,fecal traits,defecation time and abdominal distension,P <0.05.(4)Comparison of postoperative complications and symptoms: there was no significant difference between the three groups on postoperative day 3 and postoperative day 3 in pain score,P >0.05;there was a difference between the treatment group and the control group and the blank group on postoperative day 7,P <0.05,while there was no significant difference between the control group and the blank group,P >0.05.This indicated that the treatment group was better in alleviating postoperative hemorrhoid pain.There was a significant difference between the treatment group and the blank group,and between the control group and the blank group on day 3 and day 7 after surgery,P <0.05,and a statistical difference of P >0.05 between the treatment group and the control group,indicating that both the treatment group and the control group had a good effect on postoperative hemorrhoid edema and seepage.(5)lasma SP and VIP levels: there was no significant difference between the three groups in preoperative SP and VIP levels,P >0.05;the plasma SP levels of the treatment group and the control group were higher than those of the blank group on the 7th day after surgery,with statistically significant differences,P <0.05 respectively;there was no significant difference between the treatment group and the control group,P >0.05.There was no significant difference in postoperative plasma VIP level between the treatment group and the control group(P >0.05),and there was a difference between the two groups and the blank group(P <0.05).Compared with the control group before and after surgery,there was a difference in SP and VIP content between the blank group before and after surgery,P <0.05;there was no significant difference in VIP level between the treatment group and the control group before and after surgery,P >0.05;there was a difference in SP level between the treatment group and the control group before and after surgery,P <0.05.The results showed that treatment group and control group could affect the level of plasma SP and VIP,and hemorrhoid surgery could affect the expression of plasma SP and VIP.(6)Obvious side effects or adverse reactions occurred in the three groups during the treatment.Conclusion:This study shows that rhubarb mirabilite powder can prevent and treat postoperative constipation of hemorrhoids,reduce the incidence of postoperative constipation,up-regulate the expression of plasma SP and down-regulate the expression of plasma VIP,improve constipation symptoms,and have high clinical efficacy and good safety.Therefore,rhubarb mirabilite powder can be widely used in the prevention and treatment of postoperative constipation of mixed hemorrhoids. |
