Effects Of Levonorgestrel Intrauterine Contraceptive System On Prevention Of Recurrence After Hysteroscopic Endometrial Polypectomy

Objective:To explore a long-term,reliable and safe treatment to prevent the recurrence of endometrial polyps after hysteroscopic endometrial polypectomy by comparing the efficacy and adverse reactions of LNG-IUS after hysteroscopic endometrial polypectomy and hysteroscopic endometrial polypectomy.Method:Sixty patients with endometrial polyps who underwent hysteroscopic resection from September 2017 to December 2017 in Jiangxi Maternal and Child Health Hospital were collected.No treatment was performed in 30 patients(control group)and 30 patients(LNG-IUS group)were treated with levonorgestrel intrauterine contraceptive system after hysteroscopy.The amount of bleeding,operation times and ages in the two groups were observed and followed up for 3,6,12 months.The thickness of endometrium,circumferential abscission and recurrence of polyps were compared and analyzed.Results:After analysis and comparison,there were no significant difference in ages,intraoperative bleeding volumes and operation times between the two groups(P >0.05).By comparing the recurrence rates of two groups of patients,we could find that the number of recurrences in the 3rd,6th,and 12 th months after surgery was lower than that in the control group(P<0.05).There was no significant difference in menstrual changes in the third month after operation.The number of menstrual improvement was similar,but with the prolongation of time,the number of menstrual improvement in the LNG-IUS group increased,and the number of menstrual improvement in the control group gradually decreased.There were significant differences in menstrual improvement between the two groups at the 6th and 12 th months(P<0.05).There was no significant difference in the endometrial thicknessbetween the two groups before operation(t=0.277,P =0.783),the endometrial thickness of the LNG-IUS group was significantly lower than that of the control group at the 3rd month after surgery,the 6th month after surgery,and the 12 th month after surgery(P<0.05).One patient in the LNG-IUS group had ring shedding in the12 th month after surgery.Vaginal drip bleeding occurred only in 3 months after placement and only 4 cases occurred,but did not increase the total menstrual volume.Conclusions:After hysteroscopic endometrial polypectomy,the use of LNG-IUS treatment is a safe and feasible way to treat endometrial polyp.There was no significant difference in menstrual volume between the two groups at three months after surgery,with the prolongation of time,the menstrual volume in the LNG-IUS group was lower than that in the control group,and the recurrence rate was reduced,the endometrial thickness was thin,and there was little circumferential shedding.Therefore,the use of Levonorgestrel intrauterine system after hysteroscopic polypectomy can effectively reduce the recurrence of polyps and improve menstrual status.
