Correlation Between Inflammatory Response And Prognosis Of Sudden Hearing Loss

Objective: To detect the total number of white blood cells,the percentage of neutrophils,the total number of neutrophils,the total number of lymphocytes,the ratio of neutrophils to lymphocytes,the ratio of platelets to lymphocytes,the average volume of platelets,procalcitonin and antibodies to respiratory tract related pathogens in patients with sudden hearing loss.To explore the correlation between inflammatory response related indicators and prognosis of sudden hearing loss patients with different hearing curve types.That will provide guidance for clinical treatment and prediction of prognosis of sudden hearing loss.Methods: From January 2018 to February 2019,we collected materials of patients with sudden hearing loss who were hospitalized in ORL-HNS of Yijishan Hospital of Wannan Medical College,and the data of patients with simple diagnosis of vocal cord polyps or nasal septum deviation were collected.Case selection and treatment were in accordance with the Guidelines for Diagnosis and Treatment of Sudden Deafness(2015).Peripheral blood was taken within 24 hours after admission to detect the total number of white blood cells,percentage of neutrophils,total number of neutrophils,total number of lymphocytes,neutrophil/lymphocyte ratio,platelet/lymphocyte ratio,average platelet volume,respiratory virus antibody,routine blood coagulation,etc.Pure tone audiometry was performed before and after treatment,and the duration,main symptoms,accompanying symptoms and prognosis were recorded.According to different auditory curves,each group was divided into effective and ineffective groups according to the therapeutic effect.The differences of white blood cell,neutrophil,lymphocyte,platelet count,neutrophil/lymphocyte ratio,platelet/lymphocyte ratio,average platelet volume andantibodies to respiratory tract related pathogens were compared between each group.Results:(1)97 cases of sudden deafness and 87 cases of control group were collected in this study.Among the sudden deafness group,28 cases were male(28.87%)and 69 cases were female(71.13%).57 cases were left ear,38 cases were right ear,2 cases were bilateral ear,a total of 99 ears.The youngest was 10 years old and the oldest was 73 years old.The average age was40.62±14.93.The patients under 30 years old accounted for 27.84%.61.86% of them were 30-60 years old and 10.30% of them were over 60 years old.The time from onset to consultation varied from 1 day to 20 days,with an average of 6.70±5.97.In the control group,there were 50 males and 37 females,with an average age of 44.00±11.47.(2)To compare the total number of white blood cells,neutrophils,lymphocytes,platelets,mean platelet volume(MPV),neutrophil/lymphocyte ratio(NLR)and platelet/lymphocyte ratio(PLR)between sudden deafness with different hearing curve types and control group.The experimental groups:8.82±3.45,6.84±3.20,1.56±0.70,209.79±55.29,11.04±1.23,5.44±4.02,58.15±42.14;The control group: 5.75±1.37,3.28±1.03,1.86±0.52,174.10±52.31,12.48±9.85,1.90±0.92,174.10±52.31.Compared with the control group,the total number of white blood cells,the percentage of neutrophils,the total number of neutrophils,the total number of platelets and LNR in the sudden deafness group were higher than those in the control group,and the difference between the two groups was significant(P < 0.01).The total lymphocyte count,average platelet volume and PLR value of sudden deafness were lower than those of the control group,and the difference of total lymphocyte count between the two groups was also significant(P < 0.01),while the difference of MPV between the two groups was not significant(P > 0.05).(3)The results of comparison of the above-mentioned indices in patients with sudden deafness of different hearing curve types showed that:there was no significant difference among the four groups(P > 0.05).Different types of patients were compared according to their effectiveness andinefficiency.There were significant differences in WBC,N and NLR between the effective group and the ineffective group in patients with sudden deafness of low frequency descending type(P < 0.01).(4)Blood Routine Indexes of different prognosis in patients with different types of sudden deafness showed that there were significant differences in WBC,NEUT and NLR in patients with low frequency decline(P < 0.05).(5)To analyze the influence of various indicators on the therapeutic effect.Logistic regression analysis showed that that the increase of NEUT was related to prognosis(OR=0.049,95% CI:0.002-0.999).Conclusion:There are abnormalities in inflammatory reaction related indicators such as total white blood cells,neutrophils,lymphocyte,platelet,neutrophil/lymphocyte ratio(NLR),platelet/lymphocyte ratio(PLR)in patients with sudden deafness,suggesting that the abnormalities of inflammatory reaction related indicators are correlated with the occurrence of sudden deafness.For patients with different hearing curve types,WBC,NEUT and NLR in blood routine are related to the prognosis of low-frequency decline type.In addition,the results of NEUT can predict the overall effect of sudden hearing loss.
