Study On The Influence Of Pioglitazone On Peripheral Blood Inflammatory Factors In Patients With Type 2 Diabetes Complicated With Cerebral Infarction

Objective:By comparing the level of inflammatory factors,NIHSS scores and activity of daily living(ADL)scores of the observation group and the control group on the 1st and14th day,we can understand the influence of Pioglitazone on peripheral blood inflammatory factors in patients with type 2 diabetes complicated with cerebral infarction,and discuss the clinical efficacy of pioglitazone in patients with type 2 diabetes complicated with cerebral infarction.Methods:86 patients with type 2 diabetes complicated with cerebral infarction who were enrolled in our department of neurology from August 2017 to October 2018 were chosen.Among them,38 patients in the control group were given anti-platelet aggregation,lipid-lowering and hypoglycemic treatment;48 patients in the observation group were given 15 mg of Pioglitazone once daily based on the control group.The course of treatment was 14 days for both groups.5 ml of fasting venous blood was collected from all patients with type 2 diabetes complicated with cerebral infarction on the first day and the 14th day of admission.The levels of pro-/anti-inflammatory cytokines were detected by ELISA and changes in inducible nitric oxide synthase(iNOS),IL-16 and TNF-?of pro-inflammatory cytokines,and in TGF-?1,IL-4,and IL-10 of anti-inflammatory cytokines were analyzed.All patients with cerebral infarction underwent NIHSS scoring and activity of daily living(ADL)scoring on the 1st and 14th day of admission.Results:1.In all the cases,38 cases were in the control group and 48 cases were in the observation group.There were 21 males and 17 females in the control group,with an average age of60.42±9.25 and a body weight of 61.47±11.13,including 23 cases of hypertension,10cases of coronary heart disease,15 cases of hyperlipidemia,10 cases of drinking,and 14cases of smoking.There were 26 males and 22 females in the observation group,with an average age of 62.71±8.63 and a body weight of 64.88±9.96,including 30 cases of hypertension,13 cases of coronary heart disease,21 cases of hyperlipidemia,15 cases of drinking,and 19 cases of smoking.Through the t-test of measurement data,and thec~2 test of enumeration data,the differences between the two groups on age,weight,gender,hypertension,coronary heart disease,hyperlipidemia,drinking,smoking,fasting blood glucose on admission,cholesterol,triglycerides,high-density lipoprotein and low density lipoprotein had no statistical significance(P>0.05).2.The levels of anti-inflammatory factors(TGF-?1,IL-4,IL-10)on the 14th day after admission were higher than those on the first day in the same group(P<0.05).There was no significant difference in the levels of anti-inflammatory factors between the two groups on the first day of admission(P>0.05).There were significant differences in the levels of anti-inflammatory factor TGF-?1 between the two groups on the 14th day after admission(P<0.05).There was no significant difference in anti-inflammatory factors IL-4and IL-10 between the two groups(P>0.05).However,on the 14th day after admission,the mean values of anti-inflammatory factors(TGF-?1,IL-4,IL-10)in the observation group were higher than those in the control group.3.The levels of pro-inflammatory factors(iNOS,IL-16,TNF-?)on the 14th day after admission were lower than those on the first day in the same group(P<0.05).There was no significant difference in the levels of pro-inflammatory factors between the two groups on the first day of admission(P>0.05).On the 14th day after admission,the mean values of pro-inflammatory factors(iNOS,IL-16,TNF-?)in the observation group were smaller than those in the control group(P<0.05).4.The NIHSS scores on the 14th day after admission were lower than those on the first day(P<0.05),and the ADL scores were higher than those on the first day in the same group(P<0.05).There was no significant difference in the NIHSS scores and ADL scores between the two groups on the first day of admission(P>0.05).On the 14th day of admission,the NIHSS scores of the observation group were lower than that of the control group(P<0.05),and the ADL scores were greater than that of the control group(P<0.05).Conclusion:1.Regardless of the control group or the observation group,on the 14th day of admission,compared with the first day,the anti-inflammatory factors increased,the pro-inflammatory factors decreased,and the overall neurological function recovered.2.Pioglitazone can promote the expression of anti-inflammatory factor TGF-?1 in peripheral blood of patients with type 2 diabetes complicated with cerebral infarction,and inhibit the expression of pro-inflammatory factors iNOS,IL-16 and TNF-?in peripheral blood of patients with type 2 diabetes complicated with cerebral infarction.Pioglitazone can improve patients'activity of daily living and neurological deficit.
