Purpose:By detecting the levels of serum pepsinogen(PG),Helicobacter pylori(Hp)infection,and gastrin-17(G-17)levels in a large health management center in Hangzhou,we want to explore the application value of the above indicators and their combined diagnosis in the early screening of gastric cancer,and provide a scientific basis for early screening and early diagnosis of gastric cancer.Method:We selected the physical examination population of gastric function serology,carbon thirteen breath test and electronic gastroscopy at a health management center in Hangzhou from June 2017 to December 2018.There were 531 cases in all cases.We record the subject's medical examination number(ID),name(name),gender(sex),age(age)(by ID card),contact number,home address,and date of medical examination.We used enzyme-linked immunosorbent assay(ELISA)to detect serological values of PG?,PG?,and G-17.Calculate the PGR(PG?/PG?)value.We used a carbon 13 urea breath test to detect Helicobacter pylori(Hp)infection in the subject.We used electronic gastroscopy and clamped histopathology to clearly record the diagnosis of the subjects and grouped them with pathological results.The original data was entered by the medical examiner on the same day through Excel software.The data were statistically processed and analyzed using spss16.0 software.The count data were analyzed by chi-square test.The measurement data were analyzed by analysis of variance.The differences in gender and age distribution between groups were compared.PG ? level,PG ? level,PGR level,G-17 Level difference;the infection of Hp.We compared the effects of PG ?,PGR and G-17 detection and combined diagnosis in early gastric cancer screening by ROC curve comparison.Result:There were no statistical differences in gender between the six groups(P>0.05),while the age gradually increased(P<0.05).Compared with group A,the positive rate of Hp infection in each group continued to increase,and the positive rate of Hp infection in group F was nearly 4 times higher than that in group A(P<0.05).Serum PGI levels gradually increased in groups A,B,and C.After reaching a high peak in group C,they began to decrease gradually in group D,E,and F(P<0.05).Serum PGII levels were significantly elevated in each group(P<0.05).There was no significant difference between group A and group E(P>0.05).Serum PGR levels were generally lower,and the difference between group A and group B,C,and F was significant(P<0.05).There was no significant difference in the levels of PGR between group D and group E(P>0.05).Compared with group A,serum G-17 levels in groups B,C,D,E,and F gradually increased(P<0.05).The sensitivity of PGI for the diagnosis of gastric cancer was 84.30%,and the specificity was 80.30%.The sensitivity of PGR for the diagnosis of gastric cancer was 85.44% and the specificity was 50.21%.The sensitivity of G-17 for the diagnosis of gastric cancer was 87.10%,and the specificity was 58.01%.The sensitivity of PGI,PGR,and G-17 combined detection for the diagnosis of gastric cancer was 92.87%,and the specificity was 82.01%.The sensitivity and specificity of PGI,PGR and G-17 combined detection and diagnosis of gastric cancer were higher.Conclusion:With the increasing severity of gastric mucosal lesions,PGI and PGR can be used as indicators for early gastric cancer screening in a health management center in Hangzhou.Positive Hp infection can cause gastric mucosal lesions,which is closely related to gastric cancer.The sensitivity and specificity of the combined diagnosis of serum PGI,PGR and serum G-17 are high,which can provide a theoretical basis for the diagnosis of gastric cancer and provide a scientific path for early screening of gastric cancer.Serum PG detection,serum G-17 detection,carbon 13 urea breath test is expected to become a technical means for early screening of gastric cancer in a health management center in Hangzhou.
