BACKGROUND & OBJECTIVE: Mucinous epithelial ovarian carcinoma(m EOC)is derived from the germinal epithelium on the surface of the ovary and ranks second in ovarian epithelial cancer,second only to ovarian serous carcinoma.Primary ovarian mucinous carcinoma(POMC)is rare,and many ovarian mucinous tumors are initially diagnosed as primary.Many of them are transferred from other sites.The early symptoms of the patient have no obvious specificity,and generally manifest as abdominal mass,abdominal distension,abdominal pain or compression,which may cause abdominal distension due to a large amount of ascites.Poor consumption,fullness or bloating.At present,the treatment of primary ovarian mucinous carcinoma is the first comprehensive staged surgery and tumor cytoreductive surgery,after surgery and adjuvant chemotherapy.Because of the low incidence of primary ovarian mucinous carcinoma,it is rare,and there are currently few clinical graduate data.This study collected the clinical medical records of 33 patients with primary ovarian mucinous carcinoma from 2000-1-1 to 2013-1-1 in the Department of Gynecology,the Hospital and analyzed its characteristics.According to the clinical manifestations of primary ovarian mucinous carcinoma,various laboratory tests and imaging findings,surgical pathological details,postoperative treatment plan and prognosis,the clinical characteristics and prognostic characteristics were analyzed.Methods: The clinical data of 39 patients with primary ovarian mucinous carcinoma admitted to the Hospital from 2000-1-1 to 2013-1-1 were collected and the patients or their families were followed up by telephone or letter.Six patients who were lost to follow-up were reviewed.The clinical data and prognosis of 33 patients with primary ovarian mucinous carcinoma with complete follow-up data were retrospectively analyzed.The data were analyzed and statistically analyzed using SPSS20.0 software.Kaplan Meier curve method was used.Survival analysis,and using Log Rank to test,the COX regression analysis model was used to screen all the factors that affect the primary ovarian mucinous carcinoma.P<0.05 was statistically significant.RESULTS: To analyze and summarize the clinical features of primary ovarian mucinous carcinoma(including the age of onset and whether menopause,common clinical symptoms and signs,laboratory and imaging auxiliary examination,postoperative stage,tumor growth site,tumor diameter)Size,tissue differentiation and combined pleural and ascites conditions,surgical and adjuvant treatment options,survival and recurrence rates,and prognostic related factors.The age of the 33 patients was 28-77 years old,with an average age of 48.76±14.56 years.The median age was 46 years.The majority of menopausal patients were seen in 20 patients(60.60%).The general clinical symptoms are abdominal mass,abdominal pain,bloating or compression symptoms.The following abdominal masses are the most common(80%).The preoperative CA125 mean(204±66.74)IU/ML,the preoperative CA199 mean(191.7±52.82)IU/ML,and the preoperative CEA mean(13.24±5.68)IU/ML.Among them,23 patients(69.70%)had a CA125 higher than the normal range(?35 IU/ML);17 patients(51.51%)had a CA199 higher than the normal range(?35 IU/ML);CEA was higher than the normal range.There were 28 patients(84.85%)(?10 IU/ML);all patients underwent gastrointestinal endoscopy to exclude the source of gastrointestinal tumor metastasis. Due to the small sample size,this study classified patients with stage I-II as early stage,stage III-IV as late stage,20 cases(60.60%)in early stage,and 13 cases(39.39)in advanced stage.%)The early surgical procedure for ovarian mucinous carcinoma is mainly staged surgery,and the late stage is mainly tumor cytoreductive surgery.All patients underwent appendectomy.All patients underwent adjuvant chemotherapy.Thirty-three patients were followed up for 65 months to 216 months.The 5-year overall survival rate was 39.40%(13/33).The recurrence rate of patients with ovarian mucinous carcinoma was higher.Eight of the 33 patients(18.60%)relapsed,all of which were intra-abdominal recurrence.After analyzing the prognosis of patients,the single factors affecting the five-year survival rate of ovarian mucinous carcinoma were FIGO stage,degree of tissue differentiation,satisfaction of cytoreductive surgery,whether chemotherapy was full,whether a large amount of ascites was combined,and lymph node status(P<0.05).Further analysis revealed that the results showed that FIGO staging and cytoreductive surgery were satisfactory,and whether the chemotherapy treatment had a significant effect on the prognosis(P<0.05%)was an independent risk factor affecting the five-year survival rate.conclusion: 1.The age of onset of primary ovarian mucinous carcinoma is higher,more common in patients over 50 years old and menopausal.2.Abdominal mass,abdominal pain,bloating or compression symptoms are the most common clinical manifestations of primary ovarian mucinous carcinoma,of which the lower abdominal mass is the most common(80%).3.Most of the tumor markers CA125,CA199,CEA in patients with primary ovarian mucinous carcinoma are abnormal,especially CEA has the highest positive rate.4.Surgery is the main treatment for primary ovarian mucinous carcinoma.Appendectomy is performed during surgery.Postoperative chemotherapy may improve survival rate and reduce recurrence.5.The 5-year overall survival rate of patients with primary ovarian mucinous disease was:39.40%.FIGO stage,degree of tissue differentiation,satisfaction with cytoreductive surgery,whether chemotherapy is full,whether a large amount of ascites is combined,and lymph node status are risk factors for the five-year survival rate of primary ovarian mucinous carcinoma,including FIGO staging and cytoreductive surgery.Satisfaction,whether the chemotherapy is full or not is an independent risk factor affecting the five-year survival rate.
