Objectives:1.To evaluate survival results and to assess the prognostic factors in Patients with epithelial ovarian carcinoma;2.To explore the expression and clinical significance of Ki-67and WT1in epithelial ovarian carcinoma.Methods:From July2003to May2013,96Patients with epithelial ovarian carcinoma admitted to department of gynecology in The Second Affiliated Hospital, School of Medicine, zhejiang University for Primary treatment, were eligible for retrospective analysis. These Patients were given cytoreductive surgery and chemotherapy. The expression of WT1and Ki-67were detected by immunohistochemical method. The data were analyzed statistically by PASW Statistics18. Life table method was used to calculate the survival. Kaplan-Meier method was adopted to compare with the survival time. COX proportional hazards regression model was used to determine the effect of each factor on prognosis.Results:The survival rates of2and5year with96Patients of epithelial ovarian carcinomas were69.0%and31.0%respectively; Median survival time was49.0months. The prognostic factors of OS with epithelial ovarian carcinomas were related to FIGO stage, histological type, the size of residues (p<0.05). The independent prognostic factors were related to FIGO stage and the size of residues. OS of patients with serous cystadenocarcinoma was longer than with endometrioid adenocarcinoma and mixed epithelial ovarian carcinoma. Kaplan-Meier method was adopted to compare with the survival time of45serous cystadenocarcinoma in univariate analysis, showed that age, histological differential grade, amount of bleeding, neoadjuvant chemotherapy,scheme of chemotherapy treatment and the second surgery after relapse was not related to the survival time. The size of residues, the number of chemotherapy treatment course and the time of first course of chemotherapy treatment were significant Prognostic indicators. The positive expression of Ki-67in epithelial ovarian carcinoma were66.1%, and WT1with79.0%. The positive expression of Ki-67in late cancer group were significantly higher than those in early cancer group (P<0.05). OS of patients with the positive expression of Ki-67was shorter than with negative expression group (p<0.05).The positive expression of WT1in late cancer group were significantly higher than those in early cancer group (P<0.05). Size of residual tumor was related to WT1positive expression (P<0.01).OS and PFS were longer in those patients through optimal cytoreductive surgery (P<0.05).Conclusion:1.FIGO stage, Pathological type, the size of residual tumor after Primary operation, the number of chemotherapy courses, time of begining of chemotherapy treatment were significant Prognostic indicators of epithelial ovarian carcinoma.2.The second surgery has no benefit for the patient to live longer.3. The patient’s age was no related to the survival time.4. OS and PFS in the positive expression of WT1group were shorter than negative group. Expression of Ki-67and WT1gene may be related to unfavorable prognosis of patients with epithelial ovarian carcinoma. |
