Purpose Based on the analysis of inpatients in a mental hospital in Shenzhen from 2013 to 2017,this paper discusses the basic situation of patients with mental disorders,the characteristics of social demography,the structural changes of hospitalization expenses and the main influencing factors.It can provides scientific reference for health departments to reduce the economic pressure of mental illness patients,scientifically manage inpatients with mental illness and formulate relevant policies in the future.Methods The home page information of the inpatients' medical records was selected from the medical record management information system of a psychiatric hospital in Shenzhen,and the hospitalization cost analysis database was established.The descriptive method was used to analyze the basic situation and cost distribution of the inpatients.The new gray correlation method was used to analyze the cost structure,rank sum.Test and multiple linear regression models were used to analyze the influencing factors of hospitalization costs.Statistical analysis was performed using SPSS 23.0(SPSS Inc,IBM)statistical software package.Results 1.In 2013-2017,there were 34,033 inpatients,and the number of patients increased gradually,from 5,812 people per year to 7,848 people per year.The top three diseases were schizophrenia(38.26%),bipolar disorder(18.08%),and unspecific non-organic psychosis(6.54%).The first two diseases each year are schizophrenia and bipolar disorder.The third in 2013-2014 was an acute and transient psychiatric disorder,and the next three years were unspecified non-organic psychosis.Among the top ten disease diseases in 2016-2017,the more obvious growth trend is the mental and behavioral disorders caused by the use of alcohol.2.The average hospitalization days of patients with mental disorders decreased year by year,but the average hospitalization expenses and daily average costs were higher,and they were still increasing year by year.During the five-year period,the items that caused the change of hospitalization expenses were mainly in the treatment category,the integrated medical service category and the diagnosis category.3.There was no significant difference in the average hospitalization cost among patients of different genders(P>0.05).The difference of hospitalization expenses among patients of different age groups,occupations,marriage types and fee payment methods was statistically significant(All P<0.05).The difference in the average cost of different patient sources,admission routes,admission conditions,disease classification,concomitant physical illness,re-hospitalization and hospitalization within the month was statistically significant(All P<0.05).The difference in the average cost of different case classification,clinical route cases,infection,surgery,drug allergy,re-hospitalization and hospitalization days within 31 days of discharge was statistically significant(All P<0.05).Further multivariate regression analysis of confounding factors showed Hospitalization days,clinical path,with a body disease,disease classification,month readmission,drug allergy,hospitalized times,marriage,fee payment,sources of patients,case classification Conclusions 1.From 2013 to 2017,the number of inpatients with mental disorders in Shenzhen increased gradually,and the composition of mental disorders changed slightly.2.People with mental disorders average hospitalization days decline year by year,but the average hospitalization cost and average daily cost is higher,and also in increased year by year.During the five-year period,the items causing the changes of hospitalization expenses are mainly in the treatment category,comprehensive medical service category and diagnosis category,and the change direction and trend of psychiatric diagnosis and treatment mode in recent years can be seen.3.There were significant differences in hospitalization expenses between patients with different basic characteristics,disease characteristics at admission and hospitalization characteristics.Multivariate analysis showed that the first three factors that had the greatest influence on hospitalization expenses were hospitalization days,clinical pathway and physical diseases.4.Mental disease burden is heavier,health policy should be preferred,and government departments to increase mental health spending,effectively reduce the financial burden on patients and society.
